Zusammenfassung
Hintergrund
Eine medikamentöse Verzögerung der Wundheilung nach Parazentese könnte gelegentlich die Einlage von Paukenröhrchen ersetzen. In einer vergleichenden tierexperimentellen Studie haben wir untersucht, ob durch systemische Kortikoidgaben und die additive lokale Applikation von Mitomycin oder Dexamethason die Heilung einer Trommelfellperforation längerfristig gehemmt werden kann.
Methode
Bei 24 Ratten wurden jeweils beide Trommelfelle perforiert. Anschließend wurden 8 Gruppen zu je 6 Ohren gebildet, die sich hinsichtlich des gewählten Modus systemischer Dexamethasongaben (1 mg/kg Körpergewicht s.c.) bzw. der lokalen Anwendung von Dexamethason (4 mg/ml) oder gelöstem Mitomycin (2 mg/ml) unterschieden. Die Befunde wurden über 12 Wochen verfolgt.
Ergebnisse
Die Kombination von systemischen und lokalen Dexamethasongaben verzögerte die Heilungsvorgänge am perforierten Trommelfell. Mitomycingabe plus Dexamethason systemisch führte bei allen Trommelfellen zu einer Zunahme der Perforationsgröße und Destruktion des Hammergriffes.
Fazit
Für klinische Studien mit Mitomycin-Behandlung nach Parazentese ergibt sich aus den Untersuchungen, dass Patienten mit ständiger Kortikoidmedikation vorerst ausgeschlossen werden sollten.
Abstract
Background
The purpose of this study was to determine whether systemic glucocorticoid application, in combination with topical mitomycin or dexamethasone, prolongs the patency of a tympanic membrane perforation.
Method
Bilateral myringotomies were performed on 24 rats. The 48 ears were seperated into eight groups. Treatment with combined systemic and topical dexamethasone or mitomycin differed between these groups. After the first 2 weeks, the animals were checked weekly for a total of 12 weeks.
Results
A combination of systemic and topical dexamethasone delayed tympanic membrane closure after myringotomy. Perforation size increased in all tympanic membranes treated with mitomycin plus systemic dexamethasone.
Conclusion
Topical mitomycin plus systemic glucocorticoid may lead to a massive malfunction of tympanic membrane healing.
