Gutartige fibroossäre Tumoren der frontalen Schädelbasis mit intrakranieller Ausbreitung

Zusammenfassung

Bericht über 2 Patienten mit fibroossärer Läsion im Bereich der frontalen Schädelbasis mit intrakranieller Ausbreitung. Bei einer 29-jährigen Patientin handelt es sich um ein psammomatoides ossifizierendes, bei einer 37-jährigen Patientin um ein zementoossifizierendes Fibrom. Eine unspezifische Kopfschmerzsymptomatik war in beiden Fällen das führende klinische Symptom. Der Tumor und seine intrakranielle Ausdehnung wurden durch die Computer- und Magnetresonanztomographie dargestellt. Nach zunächst erfolgtem diagnostischen rhinochirurgischen Eingriff wurde der Tumor bei beiden Patienten in einer 2. Operation durch HNO-Ärzte und Neurochirurgen total entfernt.

Das ossifizierende Fibrom ist ein gutartiger Tumor, der überwiegend im Bereich der Maxilla und Mandibula lokalisiert ist. Die radikale Entfernung dieser gutartigen Tumoren wird gegenüber der Kürettage favorisiert. Aufgrund der ungewöhnlichen Lokalisation dieser seltenen Tumorentität werden die differenzialdiagnostischen Probleme, unter Berücksichtigung der aktuellen Literatur, diskutiert.

Abstract

We report on two patients with ossifying fibroma of the anterior skull base with intracranial extension. A psammomatoid ossifying fibroma was removed from a 29 year old female patient, and in a 37 year old female patient a cemento ossifying fibroma was removed. The main clinical symptom in both cases was headache. Surgery took place after diagnosis and therapy in an interdisciplinary operation with ENT colleagues. The ossifying fibroma is a benign neoplasm mainly affecting the maxilla and mandible. Complete removal is better than a curettage of the tumor.
