Rezidiv eines epitheloiden Sarkoms des Kapillitiums

Zusammenfassung

Das epitheloide Sarkom ist ein seltener maligner mesenchymaler Tumor, der klinisch aufgrund seines initial unspezifischen Erscheinungsbildes oft zu Fehldiagnosen führt. Hier berichten wir über einen 63-jährigen Patienten mit einem Rezidiv eines epitheloiden Sarkoms. Klinisch imponierte die Läsion als erythematöser, zentral ulzerierter Nodus von derber Konsistenz. Prädilektionsstellen sind die distalen Partien der oberen Extremitäten und insbesondere die Handrücken. Trotz seines langsamen Wachstums weist der Tumor klinisch ein sehr aggressives Wachstum mit häufigen Lokalrezidiven und nachfolgender lymphatischer und/oder viszeraler Metastasierung in ca. der Hälfte der Patienten auf. Histopathologisch diagnostisch wegweisend ist das Auftreten von epitheloiden und spindelzelligen Tumorzellen mit dem immunhistochemischen Nachweis von Zytokeratin und Vimentin.

Abstract

Epithelioid sarcoma is a rare soft tissue tumor that, due to its clinically unspecific features, is frequently mistaken for other benign and malignant entities. We report on a 63-year-old male presenting with a relapse of an epithelioid sarcoma. At clinical inspection, the tumor presented as an erythematous nodule with ulceration in the center. Despite its slow growth, epithelioid sarcoma should be regarded as an aggressive neoplasm exhibiting frequent local recurrences and subsequent lymphatic and visceral spreading in about half of the patients. Histopathological findings are the presence of large epithelioid and spindle cells with immunoreactivity for cytokeratin and vimentin.
