Prokeratosis Mibelli gigantea Fallbericht und Literaturübersicht

Zusammenfassung

Die Porokeratosis Mibelli ist eine seltene, hereditäre epidermale Verhornungsstörung mit verschiedenen Sonderformen und bis heute unklarer Pathogenese. Sie ist klinisch durch erythematöse Plaques mit typisch hyperkeratotischem Randwall und histologisch durch die kornoide Lamelle gekennzeichnet. Berichtet wird über eine Porokeratosis gigantea, eine morphologische Variante der klassischen Porokeratosis Mibelli. Die Pathogenese der Porokeratosis wird unter besonderer Berücksichtigung der klonalen Hypothese und der Potenz zur malignen Transformation diskutiert.

Summary

Porokeratos of Mibelli is a rare inherited disorder of epidermal keratinization, whose pathogenesis is not fully understood. The common clinical feature is a erythematous plaque surrounded by a hyperkeratotic border. The histopathologic hallmark is a parakeratotic cornoid lamella. An unusual case of porokeratosis gigantea, a morphological variant of classical porokeratosis of Mibelli, is reported. The pathogenesis, taking in account especially the clonal hypothesis and premalignant nature of porokeratosis, is discussed.
