Zusammenfassung
Nach einem randomisierten Studienansatz wurden 30 Patienten mit chronisch stationärer Psoriasis vulgaris (CSP) mit 3 verschiedenen Kombinationsschemata behandelt. Kollektiv 1 (n=10) bekam eine Dithranolmonotherapie [Dithranol (D) 2×täglich, D/D], Kollektiv 2 (n=10) Calcipotriol-Dithranol [Calcipotriol (C) morgens/Dithranol (D) abends, C/D] und Kollektiv 3 [Mometason (M) morgens/Dithranol (D) abends, M/D]. Die Behandlungsdauer betrug 4 Wochen. Die klinische Beurteilung erfolgte durch einen unabhängigen Untersucher nach dem PASI-Score sowie nach den Parametern Erythem, Schuppung und Infiltration. Das M/D-Kollektiv zeigte bereits nach der ersten Woche mit einem PASI von 5,3 einen signifikanten Unterschied zum dem D/D Kollektiv (PASI 13,22). Das C/D-Kollektiv wies mit PASI 10,5 einen geringen (nicht signifikanten) Unterschied zum D/D-Kollektiv auf. In der 4. Behandlungswoche wurden für alle Behandlungsgruppen identische PASI-Werte evaluiert. Auch 6 Wochen nach Therapieende war der PASI bei allen Gruppen gleich. Therapiebedingte periläsionale Hautirritationen waren in der M/D-Gruppe gegenüber den beiden Vergleichsgruppen deutlich geringer. Es konnte in unserem Versuchsansatz gezeigt werden, daß die antipsoriatische Effizienz der Mometason-Dithranol-Kombination der Dithranol-Dithranol-Kombination und der Calcipotriol-Dithranol-Kombination zunächst überlegen ist, im weiteren Therapieverlauf jedoch zeigen alle Gruppen identische Aktivitätsscores.
Summary
In a randomized study 30 patients with chronic stationary psoriasis were treated with 3 different topical schemes. Group 1 (n=10) received monotherapy (dithranol (D) twice a day, D/D), group 2 (n=10) calcipotriol mornings/dithranol evenings (calcipotriol (C)/dithranol (D) C/D) and 3 (mometasone (M) mornings/dithranol (D) evenings, M/D). During the therapy period of 4 weeks we documented the PASI-Score as well as infiltration, erythema and desquamation weekly. The M/D group revealed in the first week a significantly faster reduction of the PASI-score (5,3) than in the D/D group (PASI 13,22). The C/D group (PASI 10,5) show a not significantly faster reduction. After 4 weeks of treatment and after a follow period of 6 weeks there were similar PASI-Scores in all groups. There were less side-effects in the M/D group than in the others. The beginning, more anti-psoriatic effectiveness was achieved by the mometasone/dithranol combination than the other schemes. In the long term, the effects were similar.
