Zusammenfassung
In den letzten Jahren sind vornehmlich durch In-vitro-Untersuchungen eine Reihe neuer Erkenntnisse zur Eosinophilenaktivierung, zur Regulation der Eosinophilie im peripheren Blut und zur Rolle der daran beteiligten Zytokine erarbeitet worden. Danach kommt es als Ergebnis der Chemoattraktion und Aktivierung Eosinophiler durch Zytokine zur Gewebezerstörung und Förderung der lokalen Entzündungsreaktion als Folge einer lokalen Freisetzung von reaktiven Sauerstoffspezies und toxischen kationischen Granulaproteinen. Diese extrazelluläre Deposition Eosinophilen-spezifischer Granulaproteine als Ausdruck der vollständigen Eosinophilenaktivierung im Gewebe und nicht die alleinige Anwesenheit intakter Eosinophiler ist entscheidend für ihre funktionelle Effektorwirkung in situ. Zur lokalen Granuladeposition im Gewebe liegen bisher erst für die atopische Dermatitis und eine geringe Anzahl klinisch definierter Entzündungen des Hautorgans semiquantitative Untersuchungen vor, deren Ergebnisse zusammengefaßt werden. Die Kenntnis der Beteiligung Eosinophiler an der lokalen Entzündungsreaktion könnte für die Therapieplanung entzündlicher Dermatosen bedeutsam werden.
Summary
Recent in vitro-studies have substantially increased our knowledge of eosinophil activation, the regulation of eosinophilia in the peripheral blood and the role of cytokines in that process. Accordingly, chemoattraction and activation of eosinophils by cytokines results in the local release of reactive oxygen species and toxic cationic granule proteins followed by local propagation of the inflammatory reaction and tissue damage. This extracellular tissue deposition of toxic granule proteins as an expression of complete activation of eosinophils, rather than only the presence of intact eosinophil granulocytes, is crucial for their functional effect in situ. Semiquantitative studies of local eosinophil granula deposition so far have been done only in atopic dermatitis and a limited number of other inflammatory skin diseases as summarized in this review. Evaluation of the pathogenetic role of eosinophils may be of potential therapeutic importance.
