Chronische sklerodermiforme Graft-versus-Host Disease (GvHD)

Zusammenfassung.

Es wird über einen 41 Jahre alten Mann berichtet, der im Anschluß an eine allogene Knochenmarktransplantation von der HLA-identischen Schwester eine ausgedehnte sklerodermiforme GvHD entwickelte. Mit einer chronischen GvHD ist in 30% der Knochenmark-Transplantationen zu rechnen. Es handelt sich dabei um eine schwere Multisystemerkrankung, die zu einem polymorphen Krankheitsbild mit lichenoiden oder sklerodermiformen Hautveränderungen führt, aber auch Nägel, Haare und Schleimhäute betreffen kann. In Ermangelung einer kausalen Therapie sollte eine symptomatische lokale sowie eine systemische immunsuppressive Behandlung erfolgen.

Abstract.

We report on a 41-year-old patient who developed extensive chronic sclerodermatous graft-versus-host disease (GvHD) subsequent to an allogeneic bone marrow transplantation from his HLA-identical sister. Chronic GvHD is to be expected in 30% of bone marrow transplantations. It is a serious multisystem disorder which leads to a polymorphic clinical picture with lichenoid or sclerodermatous skin changes. It may also affect nails, hair and mucous membranes. In the absence of causal therapy GvHD should be managed with symptomatic local treatment and systemic immunosuppression.
