Zusammenfassung.
Wir berichten über einen 61jährigen Patienten mit akralen sklerodermiformen Hautveränderungen und sensibler Polyneuropathie, bei dem in einem Biopsat aus einem erkrankten Hautareal am Oberschenkel Borrelien-DNS mit der Polymerase-Kettenreaktion nachgewiesen werden konnte. Serologisch waren spezifische Antikörper der Klasse IgG gegen Borrelia (B.) burgdorferi nachweisbar. Der histologische Befund zeigte ein entzündliches Stadium einer Sklerose, wobei aufgrund der Beimengung von Plasmazellen im Infiltrat und der perineuralen Betonung der Infiltrate an eine Borrelieninfektion zu denken war. Immunhistochemisch konnten mit einem peroxidasemarkierten monoklonalen Antikörper gegen das B. burgdorferi Flagellin (41 kDa-Protein) von uns als spezifisch gewertete Anfärbungen in der Epidermis nachgewiesen werden. Das Ansprechen sowohl der dermatologischen als auch der neurologischen Symptomatik auf antibiotische Therapie mit Ceftriaxon unterstützt die Diagnose einer B.-burgdorferi-Infektion.
Abstract.
We report on a 61-year-old patient with sclerodermiform skin lesions of the extremities and polyneuropathy. Borrelia (B.) burgdorferi DNA was detected in lesional skin by the polymerase chain reaction. Serological testing revealed IgG antibodies to B. burgdorferi. Histology revealed an inflammatory stage of a sclerotic reaction in the lesional skin. The admixture of plasma cells and the perineural distribution of the cellular infiltrate was suggestive for a borrelia infection. Immunohistochemical staining for the B. burgdorferi flagellin (41 kDa) revealed a positive staining reaction in the epidermis of lesional skin. The improvement of both the dermatological and the neurological symptoms upon antibiotic treatment with ceftriaxone was taken as further support for the diagnosis of a. B. burgdorferi infection.
