Mycophenolatmofetil: Ein neues Immunsuppressivum in der Dermatologie und seine Einsatzmöglichkeiten

Zusammenfassung

Mycophenolatmofetil ist ein neues Immunsuppressivum, für das bisher in verschiedenen Fallberichten eine gute Wirksamkeit bei Psoriasis vulgaris und Autoimmundermatosen gezeigt werden konnte. Es handelt sich bei diesem Medikament um eine Weiterentwicklung der Mycophenolsäure, die bereits in den 70er Jahren erfolgreich bei Patienten mit Psoriasis eingesetzt wurde. Mycophenolatmofetil hat gegenüber Mycophenolsäure eine bessere Bioverfügbarkeit und kann in niedrigerer Dosierung eingesetzt werden. Hierdurch besitzt Mycophenolatmofetil ein breites therapeutisches Fenster. Derzeit wird Mycophenolatmofetil vorwiegend in der Transplantationsmedizin eingesetzt. In der vorliegenden Übersicht wird der Wirkungsmechanismus und die Pharmakologie von Mycophenolatmofetil dargestellt sowie die gegenwärtigen Erfahrungen mit diesem Medikament in der Dermatologie zusammengefaßt.

Abstract

Mycophenolate mofetil represents a new immunosuppressive drug. Several case reports have shown that mycophenolate mofetil has a good therapeutic effect in patients with psoriasis and autoimmune dermatoses. Mycophenolate mofetil is a morpholine ester of mycophenolic acid which was shown in the 1970s to be effective in patients with severe psoriasis. Mycophenolate mofetil has an increased bioavailability as compared to mycophenolic acid and thereby an improved therapeutic window. Currently, mycophenolate mofetil is indiated for the prevention of organ rejection in transplant patients. The mode of action and the pharmacology of mycophenolate mofetil are reviewed and the current clinical experience with this immunosuppressive drug for dermatological disorders is summarized.
