Subkutane Kalzifikationen nach Strahlentherapie

Zusammenfassung

Wir berichten über eine 75-jährige Patientin mit einem Spätstadium des kutanen Strahlensyndroms. Bei ihr wurde vor 32 Jahren postoperativ eine kombinierte Strahlentherapie bei einem Zervixkarzinom durchgeführt. Die aktuelle Vorstellung erfolgte wegen einer mit der 7,5-MHz-Sonographie und Magnetresonanztomographie verifizierten abszedierenden Entzündung der Kutis und Subkutis im Bereich des Bestrahlungsfeldes. Unter systemischer antibiotischer Therapie mit Cefotiam (Spizef) 2,0 g 3-mal tgl. i.v. war die Entzündung sowohl klinisch als auch magnetresonanztomographisch kontrolliert rückläufig. Als Nebenbefund fielen in der konventionellen Röntgenaufnahme, 7,5-MHz-Sonographie, Computer- und Magnetresonanztomographie ungewöhnlich stark ausgeprägte, teils groteske subkutane Verkalkungen im Bestrahlungsfeld auf. Diese Verkalkungen in der Subkutis dürften am ehesten radiogen bedingt sein.

Abstract

A 75-year-old female patient presented with late stage cutaneous radiation syndrome, following postoperative combined radiotherapy for cervical carcinoma administered 32 years ago. She was hospitalized because of a deep abscess in the radiation-exposed area on the sacrum. Extension into the subcutaneous fibrosis was verified by 7,5-MHz-sonography and nuclear magnetic resonance imaging. With intravenous cefotiam (Spizef) 2,0 g three times daily, the inflammation decreased, as seen clinically and with nuclear magnetic resonance imaging. As a secondary finding, an unusual distinctive subcutaneous calcification was diagnosed in the radiation-exposed area by means of 7,5-MHz-sonography, as well as computer- and nuclear magnetic resonance imaging. These subcutaneous calcifications are most likely to have been radiation-induced.
