Zusammenfassung
Hintergrund
Chronischer Pruritus (CP) stellt mit ca. 20,3 % ein häufiges und im Alter zunehmendes Leiden älterer Menschen dar mit signifikanter Auswirkung auf die Lebensqualität. In der Global Burden of Disease Study zählt es zu den 50 am stärksten belastenden Erkrankungen weltweit.
Ziel der Arbeit
Ziel ist es, einen handlungsbasierten Überblick über das Symptom des CP im Alter zu geben, um über eine verbesserte Differenzierung der potenziellen Ursachen des Pruritus die Versorgung dieser Patient*innengruppe zu verbessern.
Material und Methoden
Für die Literaturrecherche wurde die Datenbank PubMed herangezogen mit den Suchbegriffen „pruritus“, „elderly“ und „gerontodermatology“.
Ergebnisse
Zu den wesentlichen Ursachen des CP im Alter zählen neben dem physiologischen Alterungsprozess (Xerosis cutis, Immunseneszenz, Neuropathie) die Zunahme von potenziell pruritischen Erkrankungen im Alter (Diabetes mellitus, chronische Niereninsuffizienz) sowie die Polypharmazie. Therapeutische Optionen orientieren sich an der Ursache des Pruritus, dessen Qualität sowie den individuellen Faktoren der Patient*innen (Multimorbidität, eingeschränkte Organfunktionen), und sie orientieren sich maßgeblich an der kürzlich aktualisierten S2k-Leitlinie zur „Diagnostik und Therapie des chronischen Pruritus“.
Diskussion
Chronischer Pruritus im Alter stellt eine herausfordernde Situation für Patient*innen und Ärzt*innen dar. Neben der häufig erschwerten Ursachenfindung stellt auch die Therapie ein komplexes Zusammenspiel aus Verträglichkeit und Umsetzbarkeit dar und entscheidet somit über den Leidensweg der Patient*innen.
Abstract
Background
Chronic pruritus (CP), a frequent (20.3%) symptom in the elderly, increases with age. It has a significant impact on the quality of life, ranking among the 50 most burdensome diseases worldwide (Global Burden of Disease Study).
Objectives
The aim is to provide an overview of the symptom CP in the elderly and to improve differentiation of underlying conditions and management of this entity.
Materials and methods
A literature search in PubMed was performed, using the terms ‘pruritus’, ‘elderly’ and ‘gerontodermatology’.
Results
The main causes of CP in the elderly are the physiologic aging process (xerosis cutis, immunosenescence, neuropathy), the increase in potentially pruritic diseases with increasing age (diabetes mellitus, chronic renal failure), and polypharmacy. Therapeutic options relate to causes, severity of pruritus, and individual patient factors (multimorbidity, impaired organ function). The recently updated S2k guideline ‘Diagnosis and therapy of chronic pruritus’ is helpful.
Conclusion
CP in the elderly is challenging for both patients and physicians. Not only the difficulty of identifying the underlying cause, but the complexity of treatment and its tolerability and practicability determines these patients’ further burden.
