Zusammenfassung

Die Beibehaltung der medikamentösen Blutverdünnung durch orale Antikoagulanzien bei Eingriffen an der Haut ist mittlerweile in fast allen Fällen etablierte, leichtliniengerechte Praxis. Das früher übliche „bridging“ mit subkutanen oder intravenösen Heparinen ist heute obsolet. Trotzdem gibt es auch bei der Einnahme von neuen direkten oralen Antikoagulanzien (NOAK [neue orale Antikoagulanzien] = DOAK [direkte orale Antikoagulanzien]) noch Unsicherheiten. In dieser Übersicht analysieren wir den aktuellen Stand und geben die Empfehlungen für eine Fortführung bei Eingriffen an der Haut nach individueller Risikoabwägung.

Abstract

Current guidelines generally recommend continuation of blood thinning drugs in dermatologic surgery and the previously used “bridging” with subcutaneous or intravenous heparin is obsolete. While the guidelines are increasingly implemented in daily practice, there is still uncertainty concerning the use of the novel direct oral anticoagulants (NOAC = DOAC). In this review, we analyze current developments and formulate concise recommendations for continuation during skin surgery under consideration of individual risk.
