Zusammenfassung

Die reaktive perforierende Kollagenose (RPK) ist histologisch durch degenerierte, vertikale Kollagenbündel charakterisiert, die in einen zentralen Krater transepidermal ausgeschleust werden. Die Pathogenese ist Gegenstand kontroverser Diskussionen. Klinisch zeigen sich Papeln und Knoten mit bis zu 2 cm Durchmesser mit einem fest haftenden, zentralen Hornpfropf und gerötetem Randsaum. Sehr häufig sind Diabetes mellitus und Niereninsuffizienz als Grunderkrankung assoziiert. Wir stellen einen Patienten mit einer RPK vor, die sich nach Abheilung eines Herpes zoster am linken Stamm 2 Wochen später im selben Dermatom entwickelte und auf dieses begrenzt blieb. Unserer Kenntnis nach wurden bislang erst 5 Fälle einer RPK nach Herpes zoster beschrieben, wenngleich von einer höheren Dunkelziffer auszugehen ist. Im englischen Sprachgebrauch wird für Dermatosen, die nach Abheilen einer anderen, pathogenetisch nicht verwandten Dermatose an derselben Lokalisation entstehen, der Begriff „Wolfs isotopische Antwort“ verwendet. Wir stellen die bislang bekannten Fälle mit Herpes-zoster-assoziierter RPK vor und diskutieren den aktuellen Stand der Forschung.

Abstract

Reactive perforating collagenosis is a disease whose pathogenesis is still not fully understood. Histological findings are degenerated collagen bundles which are arranged in vertical direction penetrating the epidermis into a dome-shaped crater. Usually diabetes mellitus and renal failure can be found among patients with reactive perforating collagenosis. To date, there have been five cases described where the eruption of reactive perforating collagenosis followed herpes zoster infection. This could be a form of Wolf’s isotopic response, a term that is used for dermatoses which arise after the healing of a preexisting dermatosis. We report the sixth case of a herpes zoster-associated reactive perforating collagenosis and discuss the current literature.
