Zusammenfassung

Der pigmentierte M. Paget der Mamille ist eine seltene Erkrankung. Wir berichten über eine 60-jährige Patientin, bei der aufgrund der Klinik, Dermatoskopie, Histologie und Immunhistochemie diese Diagnose gestellt wurde. Umschriebene Pigmentierungen im Mamillenbereich beinhalten ein großes Spektrum von benignen und malignen Differenzialdiagnosen. Zur besseren klinischen Diagnostik kann die Dermatoskopie hilfreich sein. Unklare pigmentierte Läsionen der Mamille bei Frauen sowie Männern sollten immer bioptisch abgeklärt werden. Histologisch müssen selten auftretende melanozytäre Tumore vom pigmentierten M. Paget und vom pigmentierten M. Bowen abgegrenzt werden. Pagetoide intraepitheliale Zellen eines melanozytären Tumors markieren sich mit S100, HMB45 oder Melan A, sie sind negativ für Zytokeratinfärbungen. Die intraepithelialen Tumorzellen des M. Paget stellen sich mit Zytokeratin 7 dar. Der hellzellige M. Bowen ist in der Regel nicht mit Zytokeratin 7 darstellbar, und er ist zuverlässig negativ für alle melanozytären Marker.

Abstract

Pigmented Paget disease of the nipple is a rare disease. We report on a 60-year-old woman in whom this diagnosis was made based on clinical findings, dermoscopy, histology and immunohistology. Circumscribed pigmentation of the nipple includes a wide spectrum of benign and malignant differential diagnostic considerations. Dermoscopy can enhance the clinical diagnosis. Biopsies should always be performed on unclear pigmentation of the nipple in both women and men. On histology, rare melanocytic tumors must be differentiated from pigmented Paget disease and pigmented Bowen disease. Pagetoid intraepithelial cells of a melanocytic tumor are positive for S100, HMB45 or melan A, but not for cytokeratins. Intraepithelial tumor cells of Paget disease are positive for cytokeratin 7. The clear cells in Bowen diseases are usually not positive for cytokeratin 7 and are also negative for all melanocytic markers.
