Zusammenfassung

Symptomatische kardiale Metastasen des malignen Melanoms sind sehr selten. Eine 76-jährige Frau wurde 8 Jahre nach Primärexzision eines superfiziell spreitenden malignen Melanoms und 4 Jahre nach Exzision von In-transit-Metastasen wegen Ruhedyspnoe und intrathorakalen Druckschmerzes untersucht. Magnetresonanztomographie und Echokardiographie zeigten eine in das rechte Atrium hineinragende Raumforderung. Die Histologie ergab eine kardiale Melanommetastase. Wegen des Fehlens typischer klinischer Zeichen sollten gerade bei Patienten mit hohem Risiko oder bereits bekannter Fernmetastasierung auch unspezifische kardiale Symptome zum diagnostischen Ausschluss von kardialen Metastasen Anlass geben.

Abstract

Symptomatic cardiac melanoma metastases are very rare. A 76-year-old woman was admitted because of dyspnea and intrathoracic pain 8 years after surgery of a superficial spreading melanoma and 4 years after resection of in-transit metastases. MRI and echocardiography disclosed an intracavitary right atrial mass. Histologically a cardiac melanoma metastasis was found. Unspecific cardiac symptoms in a patient with elevated risk for distant metastases of melanoma should be further investigated to discover cardiac metastasis early.
