Zusammenfassung

Die sofortige oder frühe Rekonstruktion von Vollhautdefekten nach mikrographischer Chirurgie von Hautkrebs im Gesicht ist ein allgemein anerkanntes chirurgisches Prinzip. Sekundärheilungen im Gesicht werden bis heute kontrovers diskutiert. Anhand publizierter Daten und der Ergebnisse von mit dieser Technik behandelten Patienten werden Indikationen und Kontraindikationen evaluiert und für unterschiedliche anatomische Untereinheiten des Gesichtes Empfehlungen für einen sinnvollen Einsatz bei ausgewählten Patienten formuliert. Die sekundäre Wundheilung ist im Vergleich mit der chirurgischen Rekonstruktion in ausgewählten Regionen des Gesichtes eine sichere, effektive und preiswerte Alternative.

Abstract

Immediate reconstruction of full-thickness skin defects after micrographic surgical removal of facial skin cancers is a commonly accepted principle. Healing by secondary intention on the face is controversial. Considering both the literature and our own experience, we offer indications and contraindications for this approach, tailored to the various facial anatomic units. Secondary wound healing is a safe, effective and cost-saving alternative to surgical reconstruction in selected areas.
