Zusammenfassung
Das klinische Bild allergologischer Notfälle ist vielfältig. Am wichtigsten sind die häufige Anaphylaxie sowie die eher seltenen, zu Blasen führenden Überempfindlichkeitsreaktionen vom Typ des Stevens-Johnson-Syndroms bzw. der toxischen epidermalen Nekrolyse. Für den akuten Verlauf ist entscheidend, dass die durch einen exogenen Auslöser verursachte Notfallsituation möglichst rasch erkannt wird, da sonst mit schwerwiegenden Folgen bis zum Tode des Patienten zu rechnen ist. Unverzügliche Beendigung des Kontaktes mit möglichen Auslösern und symptombezogene Therapie führen meist zur folgenlosen Abheilung. Unbedingt anschließen muss sich die allergologische Diagnostik, um den Auslöser der Erkrankung zu identifizieren. Dies ist Voraussetzung langfristiger Therapie zur Vermeidung weiterer lebensbedrohlicher Reaktionen.
Abstract
The clinical presentation of allergological emergencies is heterogeneous. The most important conditions are anaphylaxis (which is frequent) and Stevens-Johnson syndrome or toxic epidermal necrolysis (which are rare). In the acute phase it is most important to recognize the exogenous elicitation of the condition as soon as possible, otherwise severe sequelae, even death of the patient, may occur. Management is based on immediate elimination of possible causative agents and symptomatic therapeutic measures. Furthermore, allergological diagnostics are necessary without delay to identify what has elicited the reaction. This information makes possible long-term treatment in order to avoid further hypersensitivity reactions.
