Abstract.
Introduction: The objectives of surgical treatment of rectal cancer are radical tumor excision, low morbidity and adequate functional outcome. The technique of total mesorectal excision (TME) has reduced the rate of local recurrences and led to an increase of ultra-low colorectal and coloanal anastomoses, which have a higher risk for anastomotic leakage. Methods: Advantages, disadvantages and consequences of staged resection for rectal cancer are discussed on the basis of international literature and our own results. Results: There are few stringent criteria for the use of diverting stomas. The decision for staged resection is often influenced by the personal experience of the surgeon. Conclusions: The occurrence of anastomotic leaks cannot be prevented by diverting ostomies, but the usually irreversible consequences regarding survival and functional outcome can be minimized.
Zusammenfassung.
Einleitung: Die chirurgische Therapie des Rectumcarcinoms hat die adäquate lokale Tumorentfernung, eine minimale Komplikationsrate und ein akzeptables funktionelles Ergebnis zum Ziel. Das Konzept der totalen mesorectalen Excision (TME) führte zu einer signifikanten Reduktion der Lokalrezivrate. Ihre Anwendung bringt eine Zunahme ultratiefer colorectaler oder coloanaler Anastomosen mit höherem Risiko für Anastomosenlecks mit sich. Methoden: Anhand von internationalen Studien und eigenen Ergebnissen werden Vor- und Nachteile sowie Folgen der zweizeitigen Chirurgie des Rectumcarcinoms in der Elektivsituation diskutiert. Ergebnisse: Es existieren wenige zwingende Richtlinien für die Indikationsstellung zur Protektion von Anastomosen durch Deviationsstomata. Analnahe colorectale oder coloanale Anastomosen werden überwiegend mit Protektionsstomien versehen, die Entscheidung für die zweizeitige Chirurgie wird aber von der subjektiven und institutionellen Erfahrung des Operateurs beeinflusst. Schlussfolgerungen: Die Anlage von Deviationsstomata kann Anastomosenleckagen nicht verhindern, jedoch irreversible Konsequenzen hinsichtlich Patientenüberleben und funktionellem Ergebnis minimieren.
