Abstract.
Introduction: Safe procedures for laparoscopic cholecystectomy demand good knowledge of the anatomy of the terminal part of the cystic artery and its variations, and also precise dissection in and arround the hepatobiliary triangle. Method: Good laparoscopic visualisation enables recognition of the variation of the cystic artery. Our observations are based on 1000 cholecystectomies. Results: We have described and named variations of the terminal part of the cystic artery. Group I comprises the five variations of the cystic artery within the hepatobiliary triangle: (a) “normal” position; (b) frontal cystic artery; (c) backside; (d) multiple; (e) short cystic artery that arises from an aberrant right hepatic artery. Group II consists of variations of the cystic artery that approach – the gallbladder beyond the hepatobiliary triangle: (a) “low-lying”; (b) transhepatic; (c) “recurent” cystic artery. Conclusion: Our classification is simple and easy to memorise and will considerably facilitate safe laparoscopic cholecystectomy.
Zusammenfassung.
Einleitung: Die genauen topographisch-anatomischen Kenntnisse der Verhältnisse des terminalen Anteils der A. cystica und ihrer Variationen sowie eine sorgfältige Dissektion im und um das hepatobiliäre Dreieck sind für die sichere Durchführung der laparoskopischen Cholecystektomie notwendig. Methoden: Nach eigenen Beobachtungen während unserer 1000 Cholecystektomien ermöglicht die exakte laparoskopische Darstellung die Erkennung der Variationen der A. cystica. Ergebnisse: Wir haben eine originelle Einteilung und Nomenklatur der Variationen des terminalen Anteils der A. cystica durchgeführt. In der ersten Gruppe finden sich 5 Variationen der A. cystica, die innerhalb des hepatobiliären Dreiecks an die Gallenblase herantreten. Das sind: (a) normale A. cystica; (b) vordere; (c) hintere; (d) mehrfache; und (e) kurze A. cystica, die von der aberranten rechten A. hepatica abgeht. Zu der zweiten Gruppe der Variationen der A. cystica gehören Arterien, die an die Gallenblase außerhalb des hepatobiliären Dreiecks herantreten. Hierzu wurden eingeordnet: (a) oberflächliche „low lying“, (b) transhepatische und (c) descendierende, „recurrente“ A. cystica. Schlußfolgerungen: Die einfache und leicht einprägsame Einteilung in zwei Gruppen mit der Nomenklatur der Lagevariationen der A. cystica, zusammen mit der Erkenntnis, daß die A. cystica nicht unbedingt innerhalb des hepatobiliären Dreiecks gefunden werden muß, erleichtert die sichere Durchführung der laparoskopischen Cholecystektomie erheblich.
