Kombinationstherapie in der Onkologie – das hepatocelluläre Carcinom

Summary.

Hepatocellular carcinoma (HCC) is being diagnosed with increasing frequency in the Western world as a consequence of the hepatitis C epidemia. Multimodal therapy is now the only effective treatment option to offer a chance of complete tumor control in HCC. Liver transplantation and liver resection, including the selection criteria and patient evaluation, are discussed. The benefits and complications of transarterial chemoembolization and cryotherapy are also reviewed. Liver transplantation is now thought to offer the best treatment option in early HCC and should be regarded as the first line of treatment in selected patients.

Zusammenfassung.

Das hepatocelluläre Carcinom der Leber (HCC) wird in der westlichen Welt vermehrt diagnostiziert, wobei dies der Epidemiologie der Hepatitis C zuzuschreiben ist. Die multimodale Therapie des HCC ist die einzige effektive Therapieoption, um eine Chance zur kompletten Tumorkontrolle zu erhalten. Die Lebertransplantation und die Lebertumorresektion mit den notwendigen Selektionskriterien und der zugehörigen Patientenevaluation werden diskutiert. Ebenso wird ein Überblick über die Vorteile und Komplikationen der transarteriellen Chemoembolisation und der Kryotherapie gegeben. Im Frühstadium des HCC wird die Lebertransplantation derzeit als die Therapie betrachtet und sollte bei selektionierten Patienten als Therapie der Wahl gelten
