Zusammenfassung
Die zytoreduktive Chirurgie ist der wesentliche Bestandteil des multimodalen Therapiekonzepts zur Behandlung von Peritonealmetastasen. Hauptindikationen sind primäre peritoneale Tumoren wie das maligne peritoneale Mesotheliom sowie sekundäre peritoneale Absiedlungen bei kolorektalen Karzinomen und Pseudomyxoma peritonei. Unter Studienbedingungen werden außerdem Patienten mit peritoneal metastasierten Magenkarzinomen, Ovarialkarzinomen oder Sarkomen behandelt.
Neben der Tumorentität sind die Tumorlast und die Tumorverteilung die wichtigsten Selektionskriterien. Die komplette Resektion aller sichtbaren Tumorherde ist bei allen Tumorentitäten der stärkste Prognosefaktor.
Eine komplette Zytoreduktion kann durch eine parietale und viszerale Peritonektomie erreicht werden. Die Operation sollte möglichst standardisiert erfolgen. Da der Tumorbefall alle 4 Quadranten des Bauchraums betreffen kann und die Präparation weitgehend extraperitoneal erfolgt, setzt der Eingriff eine besondere viszeralchirurgische und onkologische Expertise voraus.
Um die Komplikationsrate bei dieser komplexen Therapie gering zu halten, sollte die Behandlung in einem spezialisierten Zentrum erfolgen.
Die Deutsche Gesellschaft für Allgemein- und Viszeralchirurgie zertifiziert Zentren zur chirurgischen Behandlung von bösartigen Erkrankungen des Peritoneums. Die Daten der Patienten sind im zentralen HIPEC-Register dokumentiert. Der Einschluss von Patienten in laufenden Studien ist zu empfehlen.
Abstract
Cytoreductive surgery is an essential part of a multimodality treatment concept for peritoneal metastases. Indications are primary peritoneal tumors like peritoneal mesothelioma or secondaries from colorectal cancer or pseudomyxoma peritonei. Patients with gastric or ovarian carcinoma or abdominal sarcoma with peritoneal seedings can be treated within studies. Tumor entity, tumor load, and tumor distribution are the most critical issues for patient selection. Complete macroscopic cytoreduction is the strongest prognostic factor and can be achieved by parietal and visceral peritonectomy. The operation should be performed in a standardized manner. Due to possible tumor manifestation in all four quadrants of the abdomen and extensive extraperitoneal dissection, extensive surgical and oncological expertise is prerequisite. Treatment in specialized surgical oncology centers is recommended to minimize morbidity and mortality. The German Society for General and Visceral Surgery is certifying centers of competence for surgical treatment of peritoneal malignancies. Data of all patients are documented in the HIPEC register. The inclusion of patients in studies is recommended.
