Zusammenfassung:
Hintergrund
Der optimale Operationszeitpunkt zur prothetischen Versorgung bei älteren multimorbiden Patienten ist immer noch umstritten. Zur weiteren Klärung soll die durch das C-reaktive Protein (CRP) objektivierbare postoperative immunologische Reaktion herangezogen werden.
Untersuchungsmethoden
Die CRP-Werte von 122 Patienten mit komplikationsloser Implantation einer Endoprothese nach Schenkelhalsfraktur wurden prospektiv erhoben und im Zusammenhang mit der Zeit zwischen Unfalltrauma und Operation analysiert.
Ergebnisse
Es zeigen sich signifikant niedrigere CRP-Werte als Ausdruck einer geringeren immunologischen Reaktion bei der frühen prothetischen Versorgung innerhalb von 24 h nach Trauma.
Schlussfolgerung
Aufgrund der geringeren postoperativen Immunantwort bei der frühzeitigen Versorgung der Schenkelhalsfraktur mit einer Endoprothese ist hier mit einem besseren Outcome zu rechnen.
Abstract
Background
The optimal time to perform endoprosthesis of hip fractures in the elderly is still under discussion. CRP as an objective marker of postoperative inflammatory reaction should give an indication if early or late surgery is favored.
Methods
CRP values from 122 patients with no complications after hip arthroplasty of femoral neck fractures were analyzed in context with the time between the trauma and the operation.
Results
In early surgery within 24 h after trauma the CRP values were significantly lower than in delayed surgery.
Conclusion
The lower postoperative inflammatory reaction after early surgery of hip fractures provides a better outcome when treated with arthroplasty.
