Zusammenfassung

Die aneurysmatischen Erkrankungen der thorakalen und abdominellen Aorta verursachen 1–4% aller Todesfälle bei Patienten über 65 Jahren. Für den elektiven Eingriff zeigen im abdominellen Bereich sowohl die offen-chirurgischen, als auch die endovaskulären Verfahren hervorragende Gesamtergebnisse. Im thorakalen Bereich haben die neuen Stentverfahren zu einer deutlichen Senkung der Morbidität und Mortalität des Eingriffes geführt. Dennoch ist die differenzierte Indikationsstellung bei den kostenintensiven endovaskulären Verfahren hinsichtlich des noch unklaren Langzeitverhaltens notwendig, was eine zentrengebundene Expertise zwingend notwendig macht.

Abstract

One to four per cent of all deaths in patients over 65 are caused by aneurysmatic diseases of the abdominal or thoracic aorta. For elective surgery in abdominal aneurysms, open surgery and endovascular treatment both demonstrate brilliant overall results. In the thoracic aorta, new endovascular procedures have led to considerable reductions of postoperative morbidity and mortality. Nevertheless, in view of the endovascular procedure’s high cost and the still unclear long-term behaviour of the stent device, a second opinion from a specialised centre is an absolute necessity.
