Zusammenfassung
Hintergrund
Die Resultate bei bis zu 31% der konservativ behandelten Patienten mit Klavikulaschaftfraktur sind unbefriedigend. Ziel dieser Studie war, den Wert der „elastisch stabilen intramedullären Nagelung“ (ESIN) unter Verwendung eines minimal-invasiv eingebrachten, intramedullären Kraftträgers, des elastischen Titannagels (TEN), zu untersuchen.
Material und Methoden
Über einen 5-Jahres-Zeitraum wurde diese Technik bei 45 Patienten angewendet. Das Operationsverfahren wurde hinsichtlich der funktionellen Ergebnisse und Komplikationen ausgewertet.
Ergebnisse
Dreiunddreißig Fälle wurden nach einem mittleren Follow-up von 24,7±2,4 Monaten retrospektiv evaluiert. An Komplikationen traten intraoperativ 2 laterale Kortikalisperforationen und postoperativ in 8 Fällen mediale Implantatdislokationen, eine oberflächliche Wundinfektion sowie 2 Nagelbrüche auf; 12 Patienten wiesen nach Frakturheilung eine Verkürzung der betroffenen Schulter von 5 mm auf. Der mittlere Wert des Constant-Scores betrug nach mindestens 6 Monaten 94,3±2 Punkte, der des DASH-Scores 5,4±2,2 Punkte.
Schlussfolgerung
Die minimal invasive, intramedulläre Stabilisierung von Klavikulaschaftfrakturen im mittleren Drittel mit einem elastischen Titannagel führt zu funktionell guten Ergebnissen. Die Patienten sind über ein offenes Vorgehen in der Hälfte der Fälle sowie mögliche Komplikationen wie die Verkürzung des Schultergürtels, die mediale Migration und laterale Kortikalisperforation sowie Gefäß- und Nervenverletzungen aufzuklären
Abstract
Background
In up to 31% of cases, conservative treatment of midclavicular fractures leads to unsatisfactory results. The aim of this study was to define the value of minimally invasive elastic stable intramedullary nailing (ESIN) of midclavicular fractures with titanium elastic nails.
Material and Methods
Within 5 years, ESIN was perfomed in 45 patients. We studied the functional results and complications.
Results
Retrospectively we analysed the results of all patients at a mean follow-up of 24.7±2.4 months. Iatrogenic perforation of the lateral cortex occurred twice and medial migration in eight patients. Superficial skin infection developed in one. Nail breakage after fracture healing was observed twice. Twelve patients sustained clavicular shortening of ≥5 mm in relation to the intact contralateral side. After 6 months the mean constant score was 94.3±2 points, and the mean DASH score was 5.4±2.2 points.
Conclusion
Intramedullary stabilisation of midclavicular fractures with titanium elastic nails is a minimally invasive technique with good functional results. Patients must be informed about the possibility of open reduction in about half of the cases as well as shoulder asymmetry, migration of the nail, and iatrogenic nerve and vessel injury.
