Zusammenfassung
Komplikationen nach pathologischer Fraktur einer juvenilen Knochenzyste des Oberarms sind die Refraktur, das Zystenrezidiv und in seltenen Fällen der vorzeitige Verschluss der proximalen Wachstumsfuge mit nachfolgender Armlängenverkürzung. Eine Längendifferenz der unteren Extremitäten jenseits der physiologischen Grenzen bedarf auch beim Wachsenden frühzeitig eines Ausgleichs, um Fehlhaltungen des Beckens und der Wirbelsäule zu vermeiden. Der operative Ausgleich einer Längendifferenz des Humerus jedoch stellt im Wesentlichen eine kosmetische Indikation dar, da selbst bei erheblichen Differenzen funktionelle Defizite kaum auftreten. Der vorliegende Fall belegt einen langfristigen Verlauf nach einer komplizierten juvenilen Knochenzyste des proximalen Oberarms und die definitive Therapie der eingetretenen schweren Längendifferenz von 7 cm in Form der Kallusdistraktion im Erwachsenenalter. Die Ursache des vorzeitigen Fugenverschlusses und das primäre Therapiekonzept werden diskutiert.
Abstract
Complications after pathological fractures of simple bone cysts of the humerus are refracture, cyst recurrence, and in rare cases premature closure of the physeal plate with resulting shortening of the arm. Length discrepancies of the lower limbs beyond physiological limits need early compensation in children as well as adults to avoid interference with the pelvis and spine. However, surgical treatment of length differences of the humerus is usually cosmetic, because even considerable discrepancies do not lead to functional deficits. We report the long-term course of a complicated bone cyst of the proximal humerus and resolving a severe length difference of 7 cm by distraction osteogenesis in a young adult. The cause of the premature physeal closure and the primary therapeutical concept are discussed.
