Zusammenfassung
Hintergrund
Defekte in der Leiste stellen aufgrund ihrer Genese und Lokalisation immer noch eine Herausforderung dar. Zum einen entstehen solche Defekte durch die Resektion von Tumoren, zum anderen können sie aber Folge eines chirurgischen oder internistischen Eingriffs sein.
Material und Methoden
Von 2003 bis 2005 behandelten wir 11 Patienten mit Leistendefekten nach Lymphknotenmetastasen, primären Karzinomen oder Sarkomen sowie Wundheilungsstörungen nach vorangegangener Operation und Strahlentherapie. Es erfolgte eine Resektion und Defektdeckung, in 7 Fällen mit dem M.-rectus-femoris-, in 3 Fällen mit einem vertikalen M.-rectus-abdominis-Lappen, und in einem Fall erfolgte die Exartikulation im Hüftgelenk und Deckung des Defektes durch dorsale Haut und Muskelanteile aus Oberschenkel und Gesäß.
Ergebnis
Bei allen Patienten heilten die Lappen primär ein. Die Leistenregionen konnten in allen Fällen primär verschlossen werden ohne Lymphorrhö oder Sekundärinfekte.
Schlussfolgerung
Die hier vorgestellten lokalen Lappenplastiken sind gut vaskularisierte Standardlappen, die sich bei einfacher Handhabung und geringer Morbidität gut zur Defektdeckung in der Leiste eignen und den Patienten langwierige konservative Behandlungen ersparen, sie verkürzen die Krankenhausaufenthaltsdauer und vermindern auch Spätfolgen, wie Narbenkontrakturen.
Abstract
Background
Treatment of groin defects remains challenging due to their location and origin. Such defects commonly result from resection of tumours but can also occur after surgical or medical therapy.
Material and methods
From 2003 to 2005, 11 patients were treated with groin defects following radiation therapy and resection of lymph node metastasis, primary carcinoma, and sarcoma. Seven patients received wound closure with rectus femoris muscle flaps, and three had vertical rectus abdominis muscle flaps. One patient was amputated at the hip, and the defect was covered with dorsally pedicled muscle and skin flaps.
Results
All flaps healed primarily. There were no secondary infections or lymphorrhea.
Conclusion
The flaps described here are well vascularised standard flaps which are easily harvested with no major donor site morbidity. They provide sufficient coverage for the groin, reduce long-term conservative treatment, hospitalisation, and problems such as scar contracture.
