Zusammenfassung
Einleitung
Das Handgelenk ist bei der rheumatoiden Arthritis in ca. 75% der Fälle betroffen und in 12% der Fälle Ort der Erstmanifestation chronischer Polyarthritiden. Um die Zerstörung des Handgelenkes durch das Abgleiten der Handwurzel zu verhindern, haben die radiolunäre und die radioskapholunäre Arthrodese zunehmend an Bedeutung gewonnen.
Methoden
Innerhalb von 6,5 Jahren wurden 28 radiolunäre und 4 radioskapholunäre Arthrodesen bei 30 Patienten durchgeführt. Die Operationsindikation wurde bei progredientem Abgleiten und zunehmender Subluxationsstellung des Handgelenkes gestellt, wobei die vorbestehenden Destruktionen röntgenologisch maximal das Stadium III nach Larsen und Mitarbeiter erreicht haben durften.
Ergebnisse
Die im Rahmen der Nachuntersuchung (Ø 17,3 Monate) ermittelten Ergebnisse nach Partialarthrodese des Handgelenkes zeigten bei einem Großteil der behandelten Patienten bezüglich Schwellung und Schmerzen im Handgelenk Beschwerdefreiheit oder -armut und eine für den Alltagsgebrauch gute Restbeweglichkeit. Bei Patienten mit präoperativ bestehender Ulnardeviation von <15° ließ sich das weitere Zusammensintern und Verkippen des Karpus zumeist verhindern.
Schlussfolgerungen
Insgesamt haben sich die radiolunären und die radioskapholunären Arthrodesen als stabilisierende Eingriffe am Handgelenk des Rheumatikers bewährt.
Abstract
Introduction
In 75% of all cases of rheumatoid arthritis the wrist is affected and in 12% is the region of initial manifestation of this chronic inflammatory joint disease. To prevent destruction of the wrist through carpal dislocation, radiolunate and radioscapholunate arthrodeses have increased in importance.
Methods
During a 6.5-year period, 28 radiolunate and 4 radioscapholunate arthrodeses were performed in 30 patients. The indication for operation was progressive carpal translation and increasing subluxation of the wrist in which existent radiological damage had not reached more than grade III according to the classification of Larsen and co-workers.
Results
The results obtained during the follow-up study (median: 17.3 months) showed in most treated patients after partial arthrodesis of the wrist no or fewer complaints concerning swelling and pain and an acceptable remaining range of motion of the wrist in everyday life. In patients with preoperatively existing ulnar deviation less than 15° progressive carpal collapse and dislocation could be mostly prevented.
Conclusion
All told radiolunate and radioscapholunate arthrodeses successfully stabilized the wrist in patients suffering from rheumatoid arthritis.
