Zusammenfassung
Die vorliegende multizentrische Studie befasst sich mit der Erarbeitung optimaler Behandlungskonzepte bei Mittelhandfrakturen. Ausgangsdaten und Behandlungsoptionen von 1260 Patienten mit Mittelhandfrakturen wurden erfasst und hinsichtlich ihres Einflusses auf das klinisch-funktionelle und radiologische Ergebnis ausgewertet. 740 Patienten wurden nachuntersucht. Bei 487 der nachuntersuchten Patienten (66%) war nur ein Mittelhandknochen (MHK) der 3 häufigsten Lokalisationen (distale, metaphysäre und intraartikuläre Frakturen des 4. und 5. MHK, Schaftfrakturen des 2. bis 5. MHK, proximale, basisnahe Frakturen des 1. MHK) betroffen.
Das klinisch-funktionelle und radiologische Ergebnis war überwiegend sehr gut und gut. Aus der "Pfad-Analyse" der Zusammenhänge zwischen Ausgangsdaten, Behandlungsoptionen und den Ergebnissen wurden Empfehlungen für die Therapie von MHK-Frakturen abgeleitet.
Die frühfunktionelle Nachbehandlung von Mittelhandfrakturen sollte angestrebt werden. Schmerzen und Schmerzsyndrome haben einen signifikanten Einfluss auf Patientenzufriedenheit und das funktionelle Ergebnis. Sie sollten früh erkannt und adäquat therapiert werden.
Abstract
This multicenter study analyzes data on metacarpal fractures in 1260 patients to determine optimal treatment concepts. Of the 740 patients followed up, 487 (66%) presented with fractures of a single metacarpal in one of the three most frequently observed locations (distal fractures of the 4th and 5th metacarpal, shaft fractures of the 2nd to 5th metacarpal, and fractures of the base of the first metacarpal).
The overall clinical and radiological results were good to excellent. The "path" analysis used to identify baseline parameters and treatment options influencing outcome led to recommendations for metacarpal fracture treatment.
Functional protocols are predominantly applied for postoperative/post-traumatic treatment. Pain and regional pain syndromes should be prevented whenever possible and early adequate treatment should be initiated due to their significant impact on outcome.
