Zusammenfassung

Die durch die Einführung der DRGs ausgelösten Änderungen im Gesundheitssystem erfordern eine Abkehr von abteilungsbezogenen Prozessstrukturen hin zu das gesamte Krankenhaus umfassenden. Die Schnittstelle operative Medizin und Anästhesie ist daher entscheidend für eine möglichst effektive Nutzung der vorhandenen personellen und materiellen Ressourcen. Die Einführung zukunftsorientierter Informationstechnologie und geeigneter Organisationsstrukturen lässt Behandlungsabläufe besser planen und damit effektiver gestalten. Die Anästhesiologie ist daher in ein Patientenmanagementsystem sowohl für stationäre als auch ambulante Patienten mit zu integrieren. Die Etablierung von Anästhesieambulanzen bzw. -sprechstunden, die gemeinsame Erarbeitung von Prozessstrukturen in der präoperativen Vorbereitung, dem OP-Management und der postoperativen Betreuung der Patienten sind essenziell. Eine regelmäßige Kontrolle und Analyse der intensiven Zusammenarbeit anhand von OP-Leistungszahlen und Abteilungs- bzw. Krankenhauserlösen lässt Verbesserungspotenziale erkennen und die erforderlichen Steuerungselemente wirkungsvoll einsetzen.

Abstract

The changes in our health care system caused by the introduction of DRGs make it necessary for us to abandon departmental process structures in favour of total hospital orientated process structures. An interdisciplinary approach is crucial to enable the most effective use of personell and material resources. Future orientated information technology and organisational structures will enable us to process our patients effectively and efficiently from pre-admission to discharge. Anaesthesia has to be integrated into a patient management system for in- and out-patients. The essential matters for consideration are anaesthesia consulting hours, the common establishment of process structures for preoperative care, operation room management and postoperative patient care. Routine controls and analysis of the required teamwork reveal improvement potential and enable us to use the necessary control elements effectively.
