Zusammenfassung
Die Deklaration von Helsinki des Weltärztebundes (WMA) gebietet seit 1975 eindeutig die Anrufung einer Ethikkommission bei Forschung am Menschen. Trotzdem kommt es immer wieder zu Verletzungen dieser Deklaration. Nachdem sich zahlreiche englische Publikationen mit dem Thema Ethikvotum in Dissertationen und Publikationen beschäftigt haben, sollte dies erstmalig in Deutschland untersucht werden.
Insgesamt 1482 medizinische Dissertationen wurden 2013–2014 an vier ausgewählten Universitäten untersucht, ebenso wie drei deutschsprachige Publikationsorgane. Bei 543 Dissertationen wäre nach den Kriterien der Helsinki-Deklaration der Hinweis auf ein Ethikvotum erforderlich gewesen. Angegeben wurde ein Ethikvotum aber nur in 58,7 % der Fälle und noch seltener, wenn die jeweiligen Promotionsordnungen oder Merkblätter nicht auf die Notwendigkeit der Einholung eines Ethikvotums hingewiesen haben. Dissertationen aus der Pädiatrie erwähnten am häufigsten das Ethikvotum (78,6 %), dagegen war der Anteil in der Chirurgie mit 34,9 % am niedrigsten. Bei den Zeitschriften wurde am häufigsten in Der Nervenarzt ein Ethikvotum genannt (59,4 %) und am seltensten in Der Chirurg (30 %).
Die vorliegenden Ergebnisse weisen auf deutliche Lücken bei der Erwähnung von Ethikvoten in medizinischen Dissertationen und Publikationen hin. Diese Defizite ließen sich leicht schließen durch konsequentes Vorgehen der Gutachter in Promotionsverfahren bzw. der Gutachter und Herausgeber von Zeitschriften.
Abstract
Since 1975, the Declaration of Helsinki of the World Medical Association (WMA) has clearly required ethics committee approval for research into humans. Nevertheless, this Declaration is violated quite often. As many English-language publications have addressed the theme of ethics board approval in theses and other published works, it is now to be investigated in Germany for the first time.
From 2013 to 2014, a total of 1,482 medical theses at four selected universities in addition to three German-language scientific journals were reviewed. In 543 theses, reference to ethics approval would have been required according to the criteria of the Declaration of Helsinki.
However, ethics approval was stated in only 58.7% of cases, and even less frequently if the prevailing doctoral regulations or instructions did not refer to the necessity of obtaining ethics approval. Theses on pediatrics mentioned ethics approval most frequently (78.6%), whereas the proportion of surgical papers was the lowest (34.9%). Among the journals, Der Nervenarzt mentioned ethics approval most frequently (59.4%) and Der Chirurg least frequently (30%).
Our results point to significant deficits in mentioning ethics approval in medical theses and publications. These deficits could easily be compensated for by a thorough approach of the referees of doctoral regulations and by journal reviewers and editors.
