Multimorbidity and health-related quality of life among elderly persons

Zusammenfassung

Ziel der Untersuchung ist es, den Einfluss von Mehrfacherkrankungen sowie spezifischer Erkrankungen beziehungsweise gesundheitlicher Ereignisse auf die gesundheitsbezogene Lebensqualität älterer Personen zu analysieren. Grundlage dieser empirischen Studie ist ein Datensatz mit Telefoninterviews von 2120 Teilnehmern der getABI-Kohorte (76,29±4,48 Jahre; 46,3% männlich). Zur Evaluation der gesundheitsbezogenen Lebensqualität wurden der EQ-5D- und der SF-8-Fragebogen eingesetzt. Als wesentliches Ergebnis zeigt sich, dass mit zunehmender (Multi-)Morbidität die gesundheitsbezogene Lebensqualität älterer Personen signifikant absinkt. Dabei werden insbesondere körperliche Aspekte des eigenen Wohlbefindens negativer wahrgenommen. Die psychischen Bereiche sind hingegen weniger stark beeinträchtigt. Die Analyse der Einzelerkrankungen zeigt, dass vor allem kardiovaskuläre und zerebrovaskuläre Ereignisse in den letzten zwei Jahren, eine chronische periphere arterielle Verschlusskrankheit (pAVK) sowie Beeinträchtigungen des Bewegungsapparates signifikanten Einfluss auf die Lebensqualität nehmen. Informationen zur gesundheitsbezogenen Lebensqualität können helfen, den Leistungserbringern ein umfassendes Bild vom Zustand beziehungsweise der Wahrnehmung ihrer älteren, oftmals multimorbiden Patienten zu verschaffen. Infolgedessen sollte sich eine bedarfsgerechte Versorgung nicht nur auf einzelne Hauptdiagnosen konzentrieren, sondern immer auch das Gesamtbild der Morbidität – mit Blick auf die Lebensqualitätseffekte – berücksichtigen.

Abstract

Aim of this study was to analyze the effects of single diseases and multimorbidity on health-related quality of life (HRQoL) in the elderly. Based on data from telephone interwiews with the getABI cohort, empirical analyses were conducted. To evaluate HRQoL, 2,120 participants (76.29 ± 4.48 years old, 46.3% male) completed the EQ-5D and the SF-8 questionnaire. There is an inverse relationship between HRQoL and multimorbidity in the elderly. In comparison to psychological dimensions, physical domains decrease more clearly with rising morbidity. Analyzing the influence of single diseases, particularly cardiovascular and cerebrovascular diseases diagnosed in the last two years, the long-term existence of peripheral arterial disease and dysfunctions of the musculoskeletal system result in severe loss of HRQoL in older persons. Information on HRQoL can help health care providers obtain a comprehensive picture of their older and often multimorbid patients’ state and perception of health. An optimized health care process should not only focus on the individual diagnoses, but also on the extent of multimorbidity and associated HRQoL effects.

  1. Kardiovaskuläres Ereignis: Herzinfarkt, Angina pectoris, koronare Revaskularisation (zum Beispiel Bypass-Operation) oder klinisch relevante Herzinsuffizienz.

  2. Zerebrovaskuläres Ereignis: Schlaganfall, transitorische ischämische Attacke (TIA) oder Revaskularisation an den Karotiden (zum Beispiel Endarteriektomie).

  3. Durch pAVK induziertes Ereignis: Claudicatio intermittens, periphere Revaskularisation (zum Beispiel Bypass-Operation, Thrombendarteriektomie), Nekrose/Gangrän aufgrund AVK oder Amputation aufgrund AVK.

Bezüglich des signifikanten Unterschiedes zwischen den Altersgruppen ist allerdings anzumerken, dass sich einzig die Gruppe derjenigen mit zwei Ereignissen signifikant von den restlichen Personen unterscheidet. Nur diese Gruppe ist demnach signifikant älter als die restlichen homogenen Gruppen.
