Zusammenfassung
Vor über 10 Jahren wurde bei der Europäischen Arzneimittelagentur (European Medicines Agency, EMEA) das zentrale Zulassungsverfahren eingeführt. Es ermöglicht die gleichzeitige Marktzulassung von Arzneimitteln für alle Mitgliedsstaaten der Europäischen Gemeinschaft. Ursprünglich war es eine rein optionale Alternative zu den etablierten nationalen Zulassungsverfahren. Seine Bedeutung hat aber kontinuierlich zugenommen. Besonders wichtig war die Entscheidung, die Anwendung des zentralen Zulassungsverfahrens für eine Reihe wichtiger Indikationsgebiete zwingend vorzuschreiben. Parallel dazu ist auch die Bedeutung der EMEA und ihres Ausschusses für Humanarzneimittel (Committee for Human Medicinal Products, CHMP), der für die wissenschaftliche Bewertung von Arzneimitteln in zentralen Verfahren zuständig ist, deutlich gewachsen. Im vorliegenden Beitrag werden die Grundzüge des zentralen Zulassungsverfahrens vorgestellt. Es wird ausgeführt, für welche Arzneimittel das Verfahren genutzt werden muss und bei welchen es genutzt werden darf. Weiterhin wird die Vernetzung zwischen nationalen Zulassungsbehörden und CHMP bei europäischen Schiedsverfahren beleuchtet.
Abstract
The centralised procedure for marketing authorisation at the European Medicines Agency (EMEA) started more than 10 years ago, offering marketing authorisation for all member states of the European Union in a single procedure. Originally it was a purely optional alternative to the established national procedures; however, its importance has been growing ever since. The introduction of the mandatory scope for several important indications was a crucial step. The increasing importance of the centralised procedure was paralleled by the increasing weight of the EMEA and of the Committee for Human Medicinal Products (CHMP) who is responsible for scientific evaluation in the centralised procedure. This paper describes the basic principles of the centralised procedure including the definitions of the mandatory and optional scope, and how the CHMP works including its responsibility in European referrals that provide a link to national procedures.
