Zusammenfassung
Die Arbeitsepidemiologie versucht, auf den Arbeitsplatz bezogene Krankheiten zu beschreiben und die ihnen zugrunde liegenden Ursachen mit dem Ziel zu identifizieren, gefährdete Arbeitnehmer durch die Einführung primär- und sekundärpräventiver Maßnahmen vor gesundheitsschädlichen Auswirkungen des Arbeitsprozesses zu schützen. Zur Abschätzung dieser Gesundheitsrisiken stehen der Arbeitsepidemiologie eine Reihe verschiedener Studiendesigns sowie ein komplexes Instrumentarium an methodischen Verfahren zur Verfügung, die nicht durch toxikologische Untersuchungen ersetzbar sind. Der vorliegende Beitrag fokussiert auf Gesundheitsrisiken durch Gefahrstoffe am Arbeitsplatz. Eine zentrale Aufgabe ist in diesem Zusammenhang die sorgfältige Abschätzung der Exposition, d. h. die Expositionsermittlung, Expositionseinstufung und Expositionsquantifizierung. Hierfür können verschiedene Datenquellen wie die Ergebnisse von Arbeitsplatzmessungen, Registerdaten, Archivmaterialien sowie Einschätzungen von Experten und der Arbeitnehmer selbst herangezogen werden. Die Analyse biologischer Marker kann das Methodenspektrum ergänzen. Da sorgfältig durchgeführte arbeitsepidemiologische Studien eine Abschätzung des Erkrankungsrisikos unter realistischen Expositionsbedingungen erlauben, sollten ihre Ergebnisse zukünftig stärker bei der Festlegung von maximalen Arbeitsplatzkonzentrationen für Gefahrstoffe berücksichtigt werden.
Abstract
The aim of occupational epidemiology is to describe workplace-related diseases and to identify their underlying causes. Its primary goal is to protect workers from hazardous effects of the working process by applying work-related primary and secondary prevention measures. To assess health risks different study designs and a wide array of complex study instruments and methods are frequently employed that cannot be replaced by toxicological investigations. This paper primarily addresses health risks by agent exposures. In this context a central task of occupational epidemiology is careful assessment of exposure. Different data sources, such as work site measurements, register data, archive material, experts’ opinion, and the workers' personal estimates of exposure may be used during this process. In addition, biological markers can complement exposure assessment. Since thorough occupational epidemiologic studies allow assessment of disease risks under realistic exposure conditions, their results should be more frequently used to derive workplace-related threshold limit values.
