Zusammenfassung
Ehemalige Frühgeborene müssen häufig wegen Leistenhernien operiert werden. Werden diese Operationen vor Erreichen eines postkonzeptionellen Alters von 50–60 Wochen durchgeführt, so sind vor allem nach Allgemeinanästhesie postoperative respiratorische Komplikationen zu erwarten. In erster Linie handelt es sich dabei um vital bedrohliche postoperative Apnoen und Bradykardien. Zweitens werden ehemalige Frühgeborene mit schwerem Atemnotsyndrom postoperativ wiederum dem Risiko einer tagelangen maschinellen Beatmung ausgesetzt. Mit reinen Regionalanästhesietechniken lassen sich beide Komplikationen weitgehend vermeiden. Als Regionalanästhesietechniken bei ehemaligen Frühgeborenen hat sich die Spinalanästhesie etabliert. Als neue, viel versprechende Technik wird die reine Kaudalanästhesie in jüngster Zeit von einigen Anästhesisten favorisiert. Wenn eine reine Regionalanästhesie nicht indiziert oder machbar ist, scheint aufgrund theoretischer Überlegungen die Kombination einer oberflächlichen Inhalationsanästhesie mit einem Kaudalblock sinnvoll. Unabhängig von der gewählten Anästhesietechnik ist eine stationäre postoperative Überwachung angezeigt.
Abstract
Former preterm infants with postconceptual age <50–60 weeks are at risk of postoperative apnea and bradycardia when operated under general anaesthesia. In addition, after general anaesthesia with endotracheal intubation preterm infants, who had suffered from severe respiratory distress syndrome, often require prolonged postoperative mechanical ventilation. Pure regional anaesthetic techniques can avoid most of these postoperative respiratory complications. Spinal anaesthesia has been used extensively so far. Recently, pure caudal anaesthesia for this indication has become a promising alternative. If a pure regional anaesthesia technique is not indicated or feasable, the combination of light inhalational anaesthesia with a caudal block seems appropriate. Overnight cardiorespiratory monitoring is mandatory in these patients regardless of the anaesthetic technique used.
