Summary
Background and purpose
This study aimed to develop a validated survival score for elderly patients with metastatic spinal cord compression (MSCC).
Patients and methods
In all, 1,128 patients were randomly assigned to the test (n = 564) or validation group (n = 564). In the test group, ten pretreatment factors (age, gender, performance status, primary tumor, number of involved vertebrae, ambulatory status, other bone metastases, visceral metastases, interval from cancer diagnosis to radiotherapy of MSCC, time to developing motor deficits) plus the radiation regimen were retrospectively evaluated. Factors significantly associated with survival on multivariate analysis were included in the survival score. The score for each factor was determined by dividing the 6-month survival rate (%) by 10. The prognostic score represented the sum of the scores for each factor.
Results
In the multivariate analysis of the test group, age, performance status, primary tumor type, ambulatory status, other bone metastases, visceral metastases, interval from cancer diagnosis to radiotherapy of MSCC, and time to developing motor deficits were significantly associated with survival. Total scores ranged from 25 to 57 points. In the test group, 6-month survival rates were 11 % for 25–39 points, 56 % for 40–48 points, and 97 % for 49–57 points (p < 0.001). In the validation group, 6-month survival rates were 10, 53, and 94 %, respectively (p < 0.001).
Conclusion
Based on the survival scores of the test group, three prognostic groups were identified. The survival rates of the validation group were similar to the test group. This score appears reproducible and can help select the appropriate treatment for elderly patients with MSCC.

Ziel dieser Arbeit war es, einen validierten Überlebensscore für ältere Patienten mit metastatisch bedingter Rückenmarkskompression (MSCC) zu entwickeln.
Patienten und Methode
Es wurden 1128 Patienten zufällig der Test- (N = 564) oder der Validierungsgruppe (N = 564) zugeteilt. In der Testgruppe wurden 10 prätherapeutische Faktoren (Alter, Geschlecht, Allgemeinzustand, Art des Primärtumors, Anzahl befallener Wirbelkörper, Gehfähigkeit, andere Knochenmetastasen, Organmetastasen, Intervall von der Erstdiagnose der Tumorerkrankung bis zur Strahlentherapie der MSCC, Entwicklungszeit motorischer Defizite) und das Fraktionierungsschema retrospektiv evaluiert. Die Faktoren, die in der Multivarianzanalyse signifikant mit dem Überleben assoziiert waren, gingen in den Score ein. Für jeden Faktor wurde der Score ermittelt, indem die 6-Monats-Überlebensrate (%) durch 10 geteilt wurde. Der Prognosescore entsprach der Summe der Scores der einzelnen Faktoren.
Ergebnisse
In der multivariaten Analyse der Testgruppe waren Alter, Allgemeinzustand, Art des Primärtumors, Gehfähigkeit, andere Knochenmetastasen, Organmetastasen, Intervall von der Erstdiagnose der Tumorerkrankung bis zur Strahlentherapie der MSCC und die Entwicklungszeit motorischer Defizite signifikant mit dem Überleben assoziiert. Die Scores betrugen zwischen 25 und 57 Punkten. In der Testgruppe waren die 6-Monats-Überlebensraten 11 % bei 25–39 Punkten, 56 % bei 40–48 Punkten und 97 % bei 49–57 Punkten (p < 0,001). In der Validierungsgruppe waren die 6-Monats-Überlebensraten 10, 53 und 94 % (p < 0,001).
Schlussfolgerung
Unter Berücksichtigung der Überlebensscores der Testgruppe wurden drei Prognosegruppen identifiziert. Die Überlebensraten in der Validierungsgruppe waren mit denen in der Testgruppe vergleichbar. Demzufolge erscheint dieser Score gut reproduzierbar. Der Score kann bei der Wahl des geeigneten Behandlungsregimes für ältere Patienten mit MSCC hilfreich sein.
