Anatomische Gelenkflächenrekonstruktion und Wiederherstellung des Bewegungsausmaßes im Fingerendgelenk.
Indikationen
Fraktur des Fingerendglieds mit Beteiligung von ≥ 30% der Gelenkfläche.
Subluxationsstellung im distalen Interphalangeal-(DIP-)Gelenk.
Dislokation des Frakturfragments um ≥ 2 mm.
Kontraindikationen
Veraltete Frakturen.
Repositionshindernis.
Operationstechnik
Gedeckte Reposition und Fixation mit zwei perkutan eingebrachten Bohrdrähten modifiziert nach Ishiguro. Das Frakturfragment selbst wird nicht gefasst.
Weiterbehandlung
Postoperative Ruhigstellung im Gipsverband in Funktionsstellung oder in einer Fingerschiene mit Beugung im Mittelgelenk zur Entspannung des Streckapparats für 4 Wochen. Danach Entfernung der Stifte und Beginn mit aktiven Bewegungsübungen.
Ergebnisse
Zwölf Patienten mit palmarem Verrenkungsbruch des DIP-Gelenks wurden innerhalb von durchschnittlich 5 Tagen (0–8 Tage) nach Trauma operativ behandelt. Nach einem durchschnittlichen Nachuntersuchungszeitraum von 15 Monaten (9–31 Monate) betrug die durchschnittliche Beugung 79° (45–100°) bei einem durchschnittlichen Streckdefizit von 3° (0–10°). Nach den Kriterien von Warren et al. erreichten zehn Patienten ein sehr gutes Ergebnis (Streckdefizit 0–5°) und zwei Patienten ein gutes Ergebnis (Streckdefizit 6–15°). Auf der visuellen Analogskala (VAS, 0 = kein Schmerz, 10 = stärkster Schmerz) gaben elf Patienten keinen Schmerz an (VAS 0), ein Patient gab leichten Schmerz bei Belastung an (VAS 1).
Abstract
Objective
Reconstruction of the anatomic configuration of the articular surface and restoration of complete movement of the finger joint.
Indications
Fracture of the distal interphalangeal (DIP) joint involving ≥ 30% of the articular surface.
Luxation or subluxation of the distal phalanx.
Fracture-dislocation ≥ 2 mm.
Contraindications
Old fractures.
Closed reduction impossible.
Surgical Technique
Closed reduction and pin fixation modified from Ishiguro technique without penetrating the fracture fragment.
Postoperative Management
Cast fixation for 4 weeks with the finger in functional position or with the proximal interphalangeal joint flexed to relax the pulley. After 4 weeks, removal of the cast and the pin and start of active motion exercises.
Results
Twelve patients with fracture-dislocation of the DIP joint involving more than one third of the articular surface were treated with the modified extension block technique according to Ishiguro. Time from injury to surgery was 5 days (range, 0–8 days). At a mean follow-up of 15 months (range, 9–31 months), pain on the visual analog scale (VAS, 0 = no pain, 10 = maximum pain), the active range of motion, and criteria according to Warren et al. were evaluated. Eleven patients were pain-free and one patient suffered from mild pain during strenuous work. The mean active range of motion was from –3° extension (range, 0–10°) and flexion to 79° (range, 45–100°). The results according to Warren et al. were very good in ten and good in two cases.
