Die Distraktion der Haut zur Deckung von Weichteildefekten am Fuß

Zusammenfassung

Operationsziel

Deckung von Weichteildefekten der Fußsohle durch kontinuierliche Weichteildistraktion.

Indikationen

Posttraumatische Weichteildefekte, besonders in der belastungstragenden Zone der Fußsohle.

Hautdefekte nach Tumorresektion und Dekubitalulzera.

Kontraindikationen

Schlechte Weichteilverhältnisse der verbliebenen Haut. Betagte Patienten mit Diabetes mellitus oder arteriellen Verschlußkrankheiten.

Operationstechnik

Sparsames Débridement.

Fixateur externe. Rahmenkonstruktion zur temporären Ruhigstellung der Sprunggelenke. Einhaken von gebogenen Kirschner-Drähten in die Fußsohlenhaut. Befestigung der Kirschner-Drähte am Rahmen für kontinuierliche Distraktion.

Ergebnisse

Seit 1992 wurden sieben Weichteildefekte bei sechs Patienten mit der beschriebenen Technik behandelt. Mittleres Alter: 58,5 Jahre (34 bis 79 Jahre). Die Größe der Weichteildefekte schwankte zwischen 3 und 6 cm. Sehr gute Ergebnisse wurden an sechs Füßen erreicht. Bei einer betagten Frau mit Diabetes mellitus, begleitet von Infektion und Hautnekrose, war das Ergebnis schlecht, und es mußte eine Amputation vorgenommen werden.
