Zusammenfassung

Operationsziel

Chirurgische Behandlung einer persistierenden, hinteren, traumatischen Schulterluxation mit ventraler Impression des Humeruskopfes.

Indikationen

Traumatische Luxation mit einer Impression von mehr als einem Drittel der ventralen Zirkumferenz.

Verhakte, traumatische, auch verspätet erkannte, hintere Luxation.

Kontraindikationen

Traumatische, hintere Schulterluxation mit ventraler Impression.

Habituelle hintere Schulterluxation.

Operationstechnik

Hautinzision im Sulcus deltoideopectoralis.

Subkapitale Drehosteotomie, deren Korrekturwinkel der Größe der Impression plus 10o entspricht.

Osteosynthese mit einer Kinderkondylenplatte unter interfragmentärer Kompression. Frühe funktionelle Weiterbehandlung.

Ergebnisse

Zehn Patienten wurden behandelt (vier Frauen, sechs Männer; mittleres Alter 53 Jahre). Nach den Bewertungskriterien von Rowe u. Zarins erzielten ein Patient ein sehr gutes Ergebnis, fünf Patienten ein gutes, drei ein befriedigendes und ein Patient ein mäßiges (siehe Tabelle 1).
