Zusammenfassung
Operationsziel
Wiederherstellung der Schultergelenkstabilität und-funktion.
Indikationen
Rezidivierende posttraumatische vordere Schulterluxation mit sogenannter Luxationstasche.
Kontraindikationen
Fehlende sogenannte Luxationstasche bei der habituellen Schulterluxation oder multidirektionalen Instabilität.
Große Humeruskopfimpression.
Schlechte lokale Hautverhältnisse.
Schlechter Allgemeinzustand.
Operationstechnik
Anterolateraler Zugang im Sulcus deltoideopectoralis. Abtragen des kapsulären Ansatzes des Musculus subscapularis in Außenrotation des Armes. Zungenförmige ventrale Kapseleröffnung und Darstellung der Luxationstasche am Skapulahals. Verschluß der Luxationstasche durch Anheftung der Kapsel an den angefrischten Pfannenrand durch eine Knochenklammer. Kapselnaht, Reinsertion des Musculus subscapularis und weiterer schichtweiser Wundschluß. Lagerung des Armes im Gilchrist-Verband. Frühfunktionelle Nachbehandlung.
Ergebnisse
48 von 55 Patienten wurden nach durchschnittlich 2,8 Jahren (ein bis zehn Jahre) klinisch und radiologisch nachuntersucht, 20 Patienten dieser Gruppe erneut nach elf bis zwölf Jahren per Fragebogen. Die Rezidivquote betrug kurzfristig 2%, langfristig 5%. Ein sehr gutes bis gutes subjektives Ergebnis erreichten 90% der Patienten auch langfristig.
