Langzeittoxizitäten in der Behandlung des Morbus Hodgkin

Zusammenfassung

Durch kontinuierliche Therapieoptimierung in den letzten Jahren hat sich die Prognose des Morbus Hodgkin entscheidend verbessert. Diese Verbesserung des Gesamtüberlebens führt allerdings auch zu einer Zunahme einer Vielzahl möglicher Langzeitkomplikationen, deren sich der betreuende Arzt bewusst sein muss. Durch oftmals lang anhaltende Fatigue-Symptomatik, kardiopulmonale Probleme, Schilddrüsenunterfunktion sowie Schäden am gonadalen System mit Azoospermie und Ovarialinsuffizienz sind die meist jungen Patienten nachhaltig in ihrer Lebensqualität beeinträchtigt. Zusätzlich stellt die erhöhte Inzidenz von Sekundärneoplasien nach primär erfolgreicher Therapie ein erhebliches Problem dar.

Waren bisher in der Behandlung maligner Lymphome im Wesentlichen Remission bzw. Verlängerung der Überlebenszeit Hauptzielkriterien des therapeutischen Vorgehens, so rückt nun zunehmend der Aspekt der Toxizitätsverminderung bzw. -vermeidung in den Blickpunkt. Diese Entwicklung trägt dazu bei, dass Langzeittoxizitäten und Lebensqualität heute wichtige Endpunkte in modernen onkologischen Therapiestudien darstellen.

Abstract

In recent years, continuous optimization of therapy has decisively improved the prognosis of Hodgkin's disease. However, this improvement in overall survival has also led to an increase in several possible late effects which the clinician must be aware of. Due to the appearance of chronic fatigue symptoms, cardiopulmonary problems, hypothyreosis and damage to the gonadal system including azoospermia and ovarial insufficiency, the mostly young patients often suffer a persistent reduction in quality of life. In addition, the increased incidence of second malignancies following successful primary treatment presents a considerable problem.

While complete remission and prolonged survival were previously the main objectives in the therapy of malignant lymphomas, reduction or avoidance of toxicity is now becoming more and more central.. This development has led to the increasing importance of late sequelae and quality of life as endpoints in modern therapy trials in oncology.
