Zusammenfassung

Patienten mit kardiogenem Schock stellen aufgrund der kardiorespiratorischen Instabilität – neben den Herausforderungen der Therapie der Grunderkrankung – zusätzlich hohe Anforderungen an den Behandler, wenn eine Analgosedierung und Beatmung notwendig werden. Die genutzten Substanzen beeinflussen ihrerseits häufig die Hämodynamik und Oxygenierung. Der Artikel bietet eine Übersicht der gängig verfügbaren Substanzen unter Berücksichtigung der speziellen Anforderungen dieser Patienten, der Überwachung der Sedierung und der Möglichkeiten zur Optimierung der Analgosedierung.

Abstract

Patients with cardiogenic shock pose a challenge to physicians due to cardiorespiratory instability in addition to the underlying medical condition. If analgosedation and ventilation are indicated, commonly administered drugs themselves often influence hemodynamics and oxygenation. The present article provides an overview of the available substances with consideration of the patientsʼ condition, then monitoring and optimization of analgosedation.
