Kasuistik
Ein Hausarzt kontaktiert das Tropeninstitut, da ihm von dem Labor, welches er für Blutuntersuchungen beauftragt hat, gerade mitgeteilt worden ist, dass in einer Blutprobe einer seiner Patientinnen Malariaerreger ge funden worden seien. Eine parasitologische Blutuntersuchung war vom Hausarzt nicht in Auftrag gegeben worden. Seine Patientin wird umgehend informiert und stellt sich 2 h später im Tropeninstitut vor. Die 22-jährige deutsche Patientin berichtet über Kopf- und Gliederschmerzen, Fieber schübe mit Schüttelfrost und Nachtschweiß. Die Beschwerden treten seit 9 Tagen jeden 2. Tag auf. Sie berichtet, dass vor 1 Woche vom Hausarzt ein Antibiotikum (Cefuroxim) verschrieben worden sei. Am 8. Krankheitstag seien Blut abgenommen und eine HNO-ärztliche Untersuchung durchgeführt worden. Es seien eine Sinusitis festgestellt und die Behandlung auf Doxycyclin umgestellt worden. Die Patientin ist vor 5 Monaten 10 Tage in Papua-Neuguinea und 3 Wochen auf den Philippinen gewesen. In Papua-Neuguinea hatte sie Malariaprophylaxe mit Atovaquon/Proguanil regelrecht durchgeführt. Die Patientin ist bis vor 9 Tagen beschwerdefrei gewesen.
Bei der parasitologischen Blutuntersuchung wird Plasmodium vivax (Parasitämie 0,4%) nachgewiesen. Weitere auffällige Laborbefunde: Blutkörperchensenkungsgeschwindigkeit 38/h, Hämoglobin (Hb) 11,3 g/dl, Thrombozyten 21 000/ìl, Lactatdehydrogenase (LDH) 385 U/l. Die Behandlung erfolgt mit Mefloquin und nach Ausschluss eines Glucose-6-Phosphat-Dehydrogenase-(G6PD-)Mangels mit Primaquin. Die Patientin ist nach 2 Tagen bis auf leichtes Schwindelgefühl über 4 Tage beschwerdefrei. Rezidive treten nicht auf.
