Vitamin D (Colecalciferol) ist während des gesamten Lebens für die Knochengesundheit von außerordentlicher Bedeutung. Vitamin D wird konsekutiv in der Leber in 25-Hydroxy-colecalciferol sowie in der Niere in 1α,25-25-Dihydroxy-colecalciferol (Calcitriol) überführt.
Calcitriol ist das dominierende D3-Hormon, das eine große Anzahl biologischer Effekte durch Angriff am klassischen nukleären Vitamin-D-Rezeptor (VDRnuc) induziert. Es reguliert die Gentranskription in über 30 Zielorganen. Calcitriol interagiert auch mit einem putativ zellmembranständigen Rezeptor (VDRmem1,25) der für schnelle biologische Antworten verantwortlich ist.
24R,25-Dihydroxy-colecalciferol ist als ein weiteres Hormon für die Frakturheilung von essentieller Bedeutung. Es entfaltet seine biologischen Effekte durch stereospezifischen Angriff an einem zweiten membranständigen Rezeptor, VDRmem24,25
Ein weiteres D3-Hormon, das für die Regulation der Zellproliferation in der Prostata von zentraler Bedeutung ist, ist 25-Hydroxy-colecalciferol. Es wirkt hauptsächlich durch direkten Angriff am nukleären Vitamin-D-Rezeptor, z. T. jedoch auch durch seine 1α-Hydroxylierung in der Prostata.
Ältere Menschen haben häufig während des gesamten Jahres einen Mangel an 25-Hydroxy-colecalciferol. Eine Supplementierung mit täglich 400 IE Vitamin D ist erforderlich, um adäquate 25-Hydroxy-colecalciferol-Spiegel zu erzielen.
Abstract
Vitamin D (colecalciferol) is critically important for the development, growth, and maintenance of a healthy skeleton from birth until death. Vitamin D is metabolized in the liver to 25-hydroxy-colecalciferol and then in the kidney to 1α,25-dihydroxy-colecalciferol (calcitriol).
Calcitriol is the dominant D3-hormone and produces a wide array of bio-logical responses via interacting both with the classical vitamin D nuclear receptor (VDRnuc) that regulates gene transcription in over 30 target organs and with a putative cell membrane receptor (VDRmem1,25) that mediates rapid biological responses.
24R,25-Dihydroxy-colecalciferol has been shown to be an essential D3-hormone for the process of bone fracture healing. It initiates its biological responses via stereospecific binding to a second cell membrane receptor, the VDRmem24,25.
The key D3-hormone involved in the regulation of cell proliferation in the prostate is 25-hydroxy-colecalciferol. It is mainly acting directly through the nuclear vitamin D receptor but partially also through its 1α-hydroxylation in the prostate.
Elderly subjects often have mean 25-hydroxy-colecalciferol levels in the insufficiency range throughout the year. The oral dose necessary to achieve adequate serum 25-hydroxy-colecalciferol levels is a daily dose of 400 IU
