Domänenstruktur des Rezeptor-Polypeptids und Assoziation mit Begleitproteinen:
Dieser Übersichtsartikel stellt die verschiedenen Formen des Glukokortikoidrezeptors hinsichtlich ihres strukturellen Aufbaus und ihrer funktionellen Bedeutung vor. Im unaktivierten Zustand befindet sich der Rezeptor im Zytosol. Er enthält das menschliche Rezeptor-Polypeptid von 777 Aminosäuren Länge als heteromeren Komplex assoziiert mit zwei Molekülen des Hitzeschockproteins Hsp90 und einem Immunophilin. Nach Anbinden des hormonalen Liganden dissoziieren diese Zusatzproteine ab, was zur Aktivierung des Rezeptors führt.
Der Rezeptor als Transkriptionsregulator:
Nach erfolgter Aktivierung kann das Rezeptor-Polypeptid, komplexiert mit Hormon, in den Zellkern einwandern und dort regulatorisch auf Transkriptionsvorgänge einwirken. Dabei tritt es entweder als Homodimer mit Chromatin in Kontakt und veranlasst durch Anbinden an spezifische Erkennungselemente der DNA als positiver Transkriptionsfaktor eine verstärkte Expression gewebespezifischer Gene. Alternativ werden aktive Gene dadurch vermindert abgelesen, dass der Rezeptor mit Transkriptionsfaktoren wie AP-1 oder NF-κB interagiert und deren Transkriptionsaktivitäten hemmt.
Pharmakologische Erwägungen:
Auf der Ebene des Rezeptors werden die grundsätzlichen Möglichkeiten zur pharmakologischen Beeinflussung der vielfältigen Wirkungen von Glukokortikoiden diskutiert.

Domain Structure of the Receptor Polypeptide and Association with Accessory Proteins:
This review summarizes our present knowledge on the different forms of the glucocorticoid receptor emphasizing structure and functional significance. The nonactivated receptor resides in the cytoplasm. It contains the human receptor polypeptide of 777 amino acids as heteromeric complex in association with two molecules of the heat-shock protein hsp90 and one immunophilin. After binding the hormonal ligand, the receptor becomes activated by dissociation of these accessory proteins.
The Receptor Functions as Transcriptional Regulator:
The receptor polypeptide itself, complexed with hormone, moves on into the cell nucleus to there interact with chromatin and to affect transcriptional processes. By binding as homodimer to specific response elements on the DNA, the receptor functions as positive transcription factor causing increased expression of tissue-specific genes. Alternatively, the receptor interacts with transcription factors like AP-1 or NF-κB and inhibits their effects on actively transcribed genes.
Pharmacological Considerations:
The pharmacological possibilities of influencing the diverse medical actions of glucocorticoids are discussed on the level of receptors.
