Zusammenfassung.
q Hintergrund: Intoleranzreaktionen auf nichtsteroidale Antiphlogistika (NSAPs) sind häufig, und die Identifikation sicherer Ausweichpräparate stellt daher oft ein Problem in der täglichen Praxis dar. Wir untersuchten die Verträglichkeit des selektiven Cyclooxygenase-2-Inhibitors Celecoxib bei Patienten mit Intoleranzreaktionen auf NSAPs.
o Patienten und Methodik: Bei 77 Patienten (24 Männer, 53 Frauen, Alter 31–80 Jahre) mit Intoleranzreaktionen auf NSAPs wurden standardisierte Hauttestungen (Prick, Scratch, Epikutan) sowie anschließend orale, plazebokontrollierte fraktionierte Expositionstestungen unter Einschluss von Celecoxib (maximale Einzeldosis 200 mg, kumulative Tagesdosis 350 mg) durchgeführt.
o Ergebnisse: 21 Patienten wiesen anamnestisch lediglich Hautsymptome im Sinne einer Urtikaria, 25 Patienten eine isolierte Atemwegssymptomatik (Asthma) auf, bei 18 Patienten traten Haut- und Atemwegssymptome auf, und bei 13 Patienten war es zu einem anaphylaktoiden Schock gekommen. Acetylsalicylsäure war in 38 Fällen ein Auslöser der Beschwerden. In 46 Fällen verursachten mehrere NSAPs chemisch unterschiedlicher Gruppen die Symptomatik. Die orale Expositionstestung mit Celecoxib verlief bei allen 77 Probanden unauffällig.
o Schlussfolgerung: Diese Studie belegt eine fehlende Kreuzintoleranz des selektiven Cyclooxygenase-2-Inhibitors Celecoxib mit nichtselektiven NSAPs. Celecoxib ist ein sicheres Ausweichpräparat bei Intoleranz gegenüber NSAPs.
Abstract.
q Background: Adverse reactions to nonsteroidal anti-inflammatory drugs (NSAIDs) are frequent, and the need to identify a safe alternative drug is a common problem in clinical practice. We assessed the tolerability of the selective cyclooxygenase-2 inhibitor celecoxib in a group of NSAID-sensitive patients.
o Patients and Methods: 77 patients (24 males, 53 females, age 31–80 years) with a history of adverse reactions to NSAIDs underwent standardized skin prick, scratch and patch tests along with oral, placebo-controlled blinded exposure to celecoxib (maximum single dose 200 mg, cumulative daily dose 350 mg).
o Results: 21 patients had a history of cutaneous reactions only (urticaria), 25 had encountered respiratory symptoms (asthma), 18 reported cutaneous as well as respiratory symptoms, and in 13 patients an anaphylactoid shock occurred. Acetylsalicylic acid triggered symptoms in 38 patients. In 46 cases, several NSAIDs of different chemical groups caused symptoms. Oral challenge with celecoxib was tolerated by all 77 patients without adverse effects.
o Conclusion: This study demonstrates that celecoxib does not have cross-intolerance to NSAIDs. Celecoxib is a safe alternative in subjects with previous adverse reactions to NSAIDs.
