Zusammenfassung
Hintergrund: Bei der Hirnvenen- und duralen Sinusthrombose als der wichtigsten Form venöser zerebraler Durchblutungsstörungen hängen Mortalität und Morbidität ganz entscheidend von einer früheren Diagnosestellung ab. Diese gelingt nur, wenn wegweisende Befunde in Routineuntersuchungen nicht übersehen werden und durch Kenntnis der am Anfang eher unspezifischen Krankheitsbilder eine gezielte Diagnostik mit modernen bildgebenden Verfahren breit indiziert erfolgt.
Konventionelle bildgebende Verfahren: Die Mehrzahl zerebraler Sinusthrombosen ist computertomographisch (Nativ- und Kontrast-CT) zumindest als Verdachtsbefund zu diagnostizieren. Die MRT-Diagnostik kann ohne flusssensitive Sequenzen in der Frühphase der Erkrankung schwierig sein.
Aktuelle bildgebende Verfahren: Durch den ergänzenden Einsatz von CT-Angiographie, flusssensitiven MRT-Sequenzen und der MR-Angiographie kann die Diagnose in Zweifelsfällen gesichert und auf eine Kathetherangiographie fast immer verzichtet werden. Hauptgefahr moderner MRT-Verfahren ist eher die falsch positive Diagnose durch Artefakte in den MR-Angiogrammen.
Abstract
Background: In dural sinus and cerebral venous thrombosis mortality and morbidity mostly depend on an early diagnosis. This can only be achieved if knowledge exists about suspect findings in routine examinations and about the unspecific early clinical symptoms of the disease, giving rise to a specific examination procedure.
Conventional Imaging Techniques: Most cases of dural sinus thrombosis can be suspected on the basis of CT prior or after administration of contrast medium. In the early phase of the disease diagnosis based on MR imaging without the use of flow-sensitive sequences may face serious problems.
Modern Imaging Procedures: Using additional CT angiography, flow-sensitive MR sequences or MR angiography the diagnosis of cerebral venous thrombosis can be established in the majority of suspected cases without the necessity of digital subtraction angiography. The main problem of modern MR imaging is a false positive diagnosis caused by artefacts in MR angiograms.
