Zusammenfassung

Die Trikuspidalklappe und der rechte Ventrikel stehen hämodynamisch in einer engen Wechselbeziehung. Pathologischen Veränderungen der Klappe oder des Ventrikels selbst, aber auch vielfältige, darüber hinausgehende Erkrankungen können in einer Abwärtsspirale gegenseitiger Beeinflussung resultieren, die für den Patienten von prognostischer Relevanz ist. Von zentraler Bedeutung ist dabei die Entwicklung einer sekundären Trikuspidalklappeninsuffizienz. Besonders durch den Einsatz von 3‑D-Echokardiographie konnten zunehmend Veränderungen und Mechanismen identifiziert werden, die dabei eine wichtige Rolle spielen. Dieser Artikel fasst den aktuellen Wissensstand zum Wechselspiel von Trikuspidalklappe und rechtem Ventrikel mit Blick auf die Ursachen, das pathophysiologische Verständnis, die zugrunde liegenden Veränderungen und die darauf basierenden therapeutischen Ansätze zusammen.

Abstract

The tricuspid valve and the right ventricle are hemodynamically closely related. Pathological changes of the valve or of the ventricle itself and also various diseases beyond that can result in a downward spiral of mutual interference, which is of prognostic importance for the patient. The development of a functional tricuspid regurgitation is of great importance. Especially with the help of 3D-echocardiography, more and more changes and mechanisms have been identified that are crucial in this process. This article provides a review of the relationship between the tricuspid valve and the right ventricle emphasizing the current knowledge of the causes, the pathophysiological concepts, the underlying structural changes and the therapeutic approaches based on this.
