Zusammenfassung
Die dritte Version der Universellen Definition des akuten Myokardinfarkts erschien im Jahr 2012. Änderungen betreffen vor allem modifizierte Kriterien zur Befundung des Elektrokardiogramms (EKG), eine Aufwertung des Stellenwerts kardialer Bildgebung, veränderte Kriterien der Infarktsubtypen sowie die Anwendung hochsensitiver Troponin-Assays. Unter der Voraussetzung, dass objektive Hinweise für eine Myokardnekrose vorliegen, die im Zusammenhang mit einer Myokardischämie stehen, sollte die Diagnose eines akuten Myokardinfarkts gestellt werden, wenn mindestens ein Troponinwert oberhalb der 99. Perzentile einer gesunden Referenzpopulation liegt und im Verlauf ein Anstieg oder Abfall nachweisbar ist. Zusätzlich müssen Symptome der Myokardischämie, EKG-Veränderungen, Wandbewegungsstörungen im Echokardiogramm, Myokardnarben in der Magnetresonanztomographie bzw. in nuklearmedizinischen Verfahren oder ein intrakoronarer Thrombus während der Koronarangiographie oder Autopsie nachweisbar sein. Diese Arbeit bietet eine Übersicht über die Definition des akuten Infarkts und der Infarktsubtypen sowie über die leitliniengerechte Diagnostik und Therapie des akuten Koronarsyndroms. Ferner werden die Anwendung und die Interpretation von neuen hochsensitiven Troponin (hsTn)-Assays diskutiert, und es wird der Algorithmus bei Verdacht auf ein akutes Koronarsyndrom mit frühem Nachweis („rule-in“) und Ausschluss („rule-out“) eines Non-STEMI unter Verwendung eines hsTn-Tests illustriert.
Abstract
The third version of the Universal Definition of Myocardial Infarction (MI) was published in 2012. The diagnosis of acute myocardial infarction (AMI) should only be made in a clinical setting consistent with acute myocardial ischaemia when evidence of myocardial necrosis is present. The diagnostic criteria for MI are fulfilled when a rise and/or fall of cardiac biomarkers (preferentially troponins) occurs with at least one value above the 99th percentile of the upper reference limit. In addition, there should be symptoms of ischaemia, new changes in electrocardiogram (ECG), imaging evidence of a new loss of viable myocardium or new regional wall motion abnormality, or the identification of an intracoronary thrombus by angiography or autopsy. This revised definition updates previous versions by including changes to diagnostic ECG criteria, placing a higher emphasis on cardiac imaging, modifying the criteria for subtypes of MI and implementing high sensitivity cardiac troponin (cTn) assays. A guideline-based algorithm for management of patients with suspected acute coronary syndrome allowing “early rule-in” and “rule-out” of non-STEMI with high sensitivity cTn assays is also presented.
