Zusammenfassung
Die Echokardiographie spielt in der Notfallmedizin eine zentrale Rolle, da diese nicht-invasive Methode überall verfügbar ist und schnell zu wichtigen wegweisenden und therapieentscheidenden Diagnosen führt. Durch die Echokardiographie können kardiale Ursachen beim akuten Thoraxschmerz, der akuten Luftnot, der hämodynamischen Instabilität, einem neu aufgetretenen Herzgeräusch, dem Thoraxtrauma, der Frage nach kardialen Emboliequellen und beim Herzstillstand bzw. der kardiopulmonalen Reanimation geklärt werden. In der Echokardiographie zeigt die Analyse der linksventrikulären Funktion die myokardiale Ischämie und den Herzinfarkt. Die Beurteilung der rechtsventrikulären Funktion zeigt den Rechtsherzinfarkt, die Lungenembolie, den Pneumothorax und Folgen eines Thoraxtraumas. Die Beurteilung der biventrikulären Funktion differenziert verschiedene Schockformen. Die Beurteilung der Herzklappen ist eine Domäne der Echokardiographie bei allen Notfallerkrankungen mit Klappenbeteiligung. Perikardiale und parakardiale Strukturen und deren hämodynamische Auswirkungen werden ebenfalls erfasst. Es ist offensichtlich, dass aufgrund der multiplen diagnostischen Applikationen die Echokardiographie speziell im Notfall nur bei fundierter Ausbildung gut durchgeführt werden kann.
Abstract
Echocardiography plays an important role in emergency medicine because this non-invasive method is universally available and provides crucial diagnostic findings for acute decision making. The cardiac etiology in the presence of acute chest pain, acute dyspnea, hemodynamic instability or shock, new heart murmurs, chest trauma, peripheral embolism and cardiac arrest can be determined by echocardiography in the emergency scenario. The analysis of left ventricular function documents myocardial ischemia and myocardial infarction. Analysis of right ventricular function documents right heart infarction, pulmonary embolism, tension pneumothorax and sequelae of chest trauma. Echocardiography differentiates between different entities of shock. The analysis of heart valves is a domain of echocardiography. Affections of the pericardium and the hemodynamic sequelae can also be determined. It is obvious that echocardiography with its multiple diagnostic applications can only be well performed, especially in emergency medicine after in-depth education and training in this method.
