Zusammenfassung

Als mögliche Alternative zu einer dauerhaften oralen Antikoagulation mit Vitamin-K-Antagonisten bei Patienten mit Vorhofflimmern hat sich der interventionelle Verschluss des linken Vorhofohrs als nicht unterlegen in Bezug auf die neurologische Ereignisrate gezeigt. Gerade im Hinblick auf die neuen Möglichkeiten, direkte orale Faktor-II- oder Faktor-Xa-Inhibitoren zur Schlaganfallprophylaxe einsetzen zu können, ist eine individuelle Entscheidung, die das jeweilige Blutungs- und Schlaganfallrisiko mit dem periinterventionellen Risiko abwägt, erforderlich. Hier können etablierte Risiko-Scores hilfreich sein.

Abstract

As a potential alternative to long-term oral anticoagulation with vitamin K antagonists in patients with atrial fibrillation, the interventional left atrial appendage occlusion has shown to be non-inferior regarding neurological events. With the new oral direct factor II and factor Xa inhibitors playing an emerging role in stroke prophylaxis, an individual treatment strategy has to be found weighing bleeding and stroke risk against the peri-interventional complication rate based on established risk scores.
